1. The blood-brain barrier: what builds it, what crosses it, where it is deliberately missing

Every other capillary bed in the body is built to leak. The brain's is built to seal. That single difference explains most of neuropharmacology, the composition of cerebrospinal fluid, why a handful of midline nuclei can taste the blood when the rest of the brain cannot, and why a drug that works beautifully in a test tube may do nothing at all in a patient. Start with the physical structure, because everything else is a consequence of it.

The barrier is the endothelial cell, not the astrocyte

The blood-brain barrier is built by the capillary endothelial cells themselves. Four features define them. They are welded to each other by continuous tight junctions, with no gaps left between neighbouring cells. They carry no fenestrations, so there are no pores punched through the cell body. They show almost no pinocytosis, so there is essentially no vesicular shuttling of bulk fluid across the cytoplasm. And they rest on a continuous basement membrane, which is then wrapped by pericytes and by astrocyte end-feet.

Those surrounding cells matter. Pericytes sit inside the basement membrane and regulate capillary diameter and junctional tightness; astrocyte end-feet cover most of the outer surface and induce and maintain the barrier phenotype. Together with the neuron they serve, these elements make up the neurovascular unit. But the induced cell and the sealing structure are not the same thing. If you are asked what the barrier physically is, the answer is the tight junction between endothelial cells. The astrocyte is a regulator and a supporter, not the seal.

The consequence of a sealed endothelial sheet is that the paracellular route is abolished. In a normal capillary a small hydrophilic solute simply diffuses between endothelial cells and down its gradient into the interstitium. In the brain there is no between. Every solute that enters nervous tissue has to pass through the endothelial cell: into the luminal membrane, across the cytoplasm, out of the abluminal membrane. That forces a hard selection. Small lipophilic molecules dissolve through the lipid bilayer and move freely. Everything polar waits for a carrier. Everything large waits forever.

Rule Barrier tightness is set by the junction. Wherever the junctions are leaky, the barrier is leaky, and wherever they are continuous, entry must be transcellular.

Rule Crossing a sealed endothelium means going through the cell, so there are exactly two ways in: lipid solubility, or a transporter.

Three capillary wall types, and why only one of them seals

It is worth holding the three capillary architectures side by side, because the distractor that catches people is almost always a wall type borrowed from another organ.

Capillary type	Wall features	Where found
Continuous, with tight junctions	Continuous tight junctions, no fenestrations, minimal pinocytosis, continuous basement membrane, pericytes and astrocyte end-feet	Brain and spinal cord generally; this is the blood-brain barrier
Fenestrated	Pores roughly 60 to 80 nanometres wide punched through the endothelial cytoplasm; basement membrane intact	Renal glomerulus, gut mucosa, endocrine glands, choroid plexus, circumventricular organs
Sinusoid, discontinuous	Wide channels with large interendothelial gaps and a discontinuous or absent basement membrane, so whole cells and plasma proteins pass	Liver, spleen, bone marrow

Trap Ordinary brain capillaries are sometimes said to lack a basement membrane. They do not. The basement membrane is continuous, and it is reinforced by pericytes and astrocyte end-feet as part of the neurovascular unit. What they lack is fenestrations.

Trap Fenestrated and sinusoidal are not synonyms. A fenestration is a pore through a single endothelial cell, about 60 to 80 nanometres, and it still filters by size. A sinusoid is a gap between cells wide enough for erythrocytes and albumin, which is why liver, spleen and marrow can traffic whole cells across the wall and brain cannot.

What gets in: lipid solubility, carriers, and the pumps that push back out

Small uncharged lipophilic molecules need no help at all. Steroid hormones are the clean example: estrogen dissolves straight through the endothelial membrane and reaches brain tissue without any transport machinery. Many anaesthetics, ethanol, nicotine and the sedating first-generation antihistamines behave the same way, which is exactly the point of their central effects.

Polar nutrients get in only because the brain builds carriers for them. Glucose crosses on GLUT1 by facilitated diffusion. Large neutral amino acids cross on the large neutral amino acid transporter, and this is the single most clinically exploited carrier in the nervous system. Dopamine itself is a charged catecholamine and cannot cross the barrier, so giving dopamine orally does nothing for Parkinson disease. Its immediate amino acid precursor L-dopa is a large neutral amino acid, rides the carrier into the brain, and is then decarboxylated to dopamine inside the central nervous system. That is the prodrug trick in its purest form: if the active molecule cannot cross, deliver a precursor that can.

Two groups stay out. Large, charged or protein-bound molecules have neither lipid solubility nor a carrier. Proteins are the extreme case: too large and too hydrophilic to slip between tight junctions and far too hydrophilic to dissolve through membrane lipid. The numbers make the exclusion concrete. Cerebrospinal fluid protein is only about 34 mg/dL, against roughly 6500 mg/dL in plasma, a difference of more than two orders of magnitude that exists purely because the barrier holds protein back.

Second, there is active expulsion. Efflux pumps in the luminal membrane, above all P-glycoprotein, recognise lipophilic xenobiotics that have dissolved into the endothelial cell and throw them back into the blood before they ever reach brain tissue. This is not a passive failure to enter; it is energy-dependent rejection, and it is why lipid solubility alone does not guarantee central penetration. Pharmacology exploits it deliberately. Fexofenadine is a second-generation antihistamine built to be polar and to be a P-glycoprotein substrate, so it blocks peripheral H1 receptors and stays out of the brain. The clinical payoff is an antihistamine that does not sedate.

Route of entry	Examples	Why it works or fails
Lipid diffusion through the membrane	Estrogen and other steroid hormones, small lipophilic drugs	Small and uncharged, so they dissolve through the bilayer with no carrier needed
Carrier-mediated transport	Glucose on GLUT1; L-dopa on the large neutral amino acid transporter	Polar molecules the brain must have, so a dedicated transporter is expressed
Excluded by size and charge	Plasma proteins, large charged molecules, dopamine itself	No paracellular gap and no lipid solubility, so there is no path through the cell
Actively pumped back out	Fexofenadine and other P-glycoprotein substrates	Lipophilic enough to enter the endothelial cell but ejected by efflux pumps before reaching tissue

Memory hook Lipophilic in, or carried in. Everything else is out, and some of what gets in is thrown back.

Memory hook Tight junctions, no fenestrations, no pinocytosis. Three negatives and one weld, and that is the whole barrier.

High yield L-dopa treats Parkinson disease and dopamine does not, because only the large neutral amino acid precursor has a carrier at the barrier. Fexofenadine does not sedate because it is polar and a P-glycoprotein substrate. Both facts are the same principle read in opposite directions.

The circumventricular organs: sensing sites built with the barrier left open

A sealed barrier is a problem for any neuron whose job is to monitor the blood. The solution is anatomical: a small number of midline sites around the ventricles are built with fenestrated capillaries and an incomplete barrier, so their neurons can sample plasma directly. These are the circumventricular organs, and because they are positioned next to the structure that needs the information, their location predicts their function.

Circumventricular organ	Location	What it senses and drives
Area postrema	Floor of the fourth ventricle at the caudal medulla	Circulating toxins, uremic and metabolic products, and drugs including chemotherapy agents and opioids; drives the vomiting reflex as the chemoreceptor trigger zone
Subfornical organ	Anterior wall of the third ventricle, hardwired to the hypothalamus	Plasma osmolality and angiotensin II; drives thirst and vasopressin release
Organum vasculosum of the lamina terminalis	Anterior wall of the third ventricle, hardwired to the hypothalamus	Osmolality and circulating interleukins and other pyrogens; generates fever, anorexia and sickness behavior
Pineal gland	Posterior roof of the third ventricle	Hormonal output of the circadian clock; releases melatonin to signal darkness

Work through the logic rather than memorising the list. The area postrema sits in the floor of the fourth ventricle, in the caudal medulla, next to the vomiting circuitry of the brainstem. Give it a leaky barrier and it becomes the chemoreceptor trigger zone: it smells poison in the blood and makes you vomit. This is why cytotoxic chemotherapy and opioids are emetic, why uremia nauseates, and why antiemetics that act centrally target a site that is technically outside the barrier.

The subfornical organ and the organum vasculosum of the lamina terminalis sit on the anterior wall of the third ventricle, directly adjacent to the hypothalamus. Give them a leaky barrier and they become the hypothalamus's blood sensors: they read plasma osmolality and angiotensin II and drive thirst and vasopressin release, which is water and ion balance, and they read circulating pyrogens and generate fever with the rest of the sickness response. The pineal gland, also a circumventricular organ, is the output rather than the input: it exports melatonin into the blood.

Rule A leaky barrier marks a sensing site, and the structure next door names what is being sensed. Fourth ventricle floor next to vomiting circuitry means toxins; third ventricle anterior wall next to the hypothalamus means osmolality, angiotensin and pyrogens.

Trap Do not reverse the direction of the circumventricular organs. Keeping drugs out is the job of the intact barrier with its tight junctions and efflux pumps. A circumventricular organ does the opposite: it lets blood-borne signals in.

Trap Keep the midline functions separate. Vomiting to blood-borne toxins is the area postrema in the fourth ventricle floor. Melatonin is the pineal gland. Thirst, vasopressin and fever are the third-ventricle anterior wall pair. Cerebrospinal fluid secretion belongs to none of them: that is the specialized ependymal cells of the choroid plexus in the lateral, third and fourth ventricles. And vision belongs to the retina, optic pathway and occipital cortex, which are not circumventricular organs at all.

One structural link is worth drawing now because it reappears later. The choroid plexus is also built on fenestrated capillaries, so blood-derived solutes reach the plexus stroma freely. What stops them there is not the endothelium but the tight junctions of the specialized ependymal cells sitting on top, which form the blood-cerebrospinal fluid barrier. Same principle, different cell layer: find the tight junction and you have found the barrier.

2. Fuel: a 2 percent organ with a 20 percent appetite and no reserve

The brain is about 2 percent of body weight, yet it consumes roughly 20 percent of resting oxygen and about 20 percent of calories, and it takes a similarly large share of cardiac output. That tenfold mismatch between mass and metabolic claim is the single most important number in cerebrovascular physiology, because everything about the fragility of the brain follows from it.

Where the energy goes

Neurons do not spend their energy on growth or contraction. They spend it on the Na-K ATPase, restoring the sodium and potassium gradients that every action potential degrades. A neuron that has just fired has to pump sodium back out and potassium back in before it can fire again, and the pump runs on ATP. Synaptic transmission adds further cost: neurotransmitter reuptake, vesicle cycling and the maintenance of calcium gradients all draw on the same supply. Electrical signalling is metabolically expensive by design, and the brain signals constantly.

The second half of the problem is that neurons store essentially nothing. Brain glycogen is negligible and what little exists is confined mostly to astrocytes, not neurons. There is no local fat depot that neurons can oxidise. There is no oxygen reserve beyond what is dissolved in the blood already in transit. The brain is therefore wholly dependent on continuous arterial delivery, moment to moment, with no buffer whatsoever against interruption.

Flow stops Arterial delivery of oxygen and glucose ceases.
About ten seconds Consciousness is lost, because cortical ATP supply fails almost immediately.
Four to six minutes Irreversible neuronal damage begins, as pumps fail, ion gradients collapse and excitotoxic cascades start.
Beyond that window Infarction is established and the territory is lost, which is why stroke is managed as a time-critical emergency.

High yield Two percent of body weight, 20 percent of resting oxygen and calories, unconscious in about ten seconds, irreversible damage in four to six minutes. Those four numbers are the reason acute stroke care is organised around minutes rather than hours.

Trap Do not confuse the numerator with the denominator. Two percent is the brain's share of body weight, the baseline the comparison is made against, not its metabolic share. Twenty percent is the oxygen and calorie claim. Fifty percent is far too high for any single organ at rest, and five percent would be a proportionate figure for a larger and much less active organ, badly underestimating neuronal energy demand.

Glucose is the obligate fuel

Glucose is the brain's primary and obligate substrate. It crosses the barrier by facilitated diffusion on GLUT1, which means it needs no insulin at all; brain glucose uptake is insulin-independent, so it continues in the fasting state and in untreated diabetes alike. Once inside, glucose funds the Na-K ATPase work described above.

Because brain glycogen is negligible, supply depends entirely on continuous arterial delivery. Hypoglycemia therefore behaves like a flow problem: confusion appears quickly and coma follows within minutes. The clinical lesson is that a patient with an acute neurological deficit gets a glucose measurement immediately, because hypoglycemia is both a mimic of stroke and a reversible cause of coma.

Two supplementary fuels exist, and each has its own window. Lactate shuttled from astrocytes supplements glucose during intense neuronal activity, when local demand briefly outruns glucose delivery; astrocytes take up glucose, glycolyse it and hand lactate to neurons, which oxidise it. In prolonged fasting, the ketone bodies beta-hydroxybutyrate and acetoacetate are carried across the barrier on monocarboxylate transporters and can cover a large share of cerebral demand, which is what allows survival through extended starvation without catastrophic neuronal loss.

Fuel	Access to the brain	Role
Glucose	Facilitated diffusion on GLUT1, insulin-independent	Primary and obligate fuel; supply is continuous because stores are negligible
Lactate	Shuttled from astrocytes to neurons locally	Supplements glucose during intense neuronal activity
Ketone bodies	Beta-hydroxybutyrate and acetoacetate carried across the barrier	Cover much of cerebral demand during prolonged fasting
Fatty acids	Largely albumin-bound and not meaningfully transported in	Not used by neurons; the liver converts them to ketones the brain can use
Amino acids	Cross on carriers such as the large neutral amino acid transporter	Neurotransmitter precursors and protein building blocks, not a bulk energy source

Trap Long-chain fatty acids are not a brain fuel. They are largely albumin-bound, are not meaningfully transported across the barrier, and are not oxidised by neurons. The liver oxidises them and converts them into ketones, and it is the ketone that the brain uses. Writing "fatty acids" as the fasting brain fuel skips the liver step that makes the statement true.

Trap Amino acids do cross on carriers, which makes them tempting as an answer, but their function is to build proteins and supply neurotransmitter precursors. They are not a bulk energy source. And glycogen stores are not an answer either: brain glycogen is tiny and mostly astrocytic, which is precisely why a few minutes without glucose or oxygen already causes damage.

Memory hook Glucose on GLUT1, no insulin needed. Backups in order of time scale: astrocyte lactate for the next seconds, ketones for the next days. Fatty acids never.

Rule An organ with a tenfold mismatch between mass and metabolic rate and no stored fuel cannot tolerate interruption. Every cerebrovascular syndrome in this topic is a variation on how that interruption happens.

3. Arterial inflow: two systems that meet in a ring

The brain is fed by exactly two inflows, and they meet at its base. Every stroke question starts by asking which of the two is involved, because the territories, the syndromes and often the mechanism differ. Get the two-system frame first and the detail hangs off it.

The anterior circulation: internal carotid

The anterior circulation arises from the internal carotid arteries. Each enters the skull through the carotid canal in the petrous temporal bone, runs forward through the cavernous sinus, and then gives, in order, the ophthalmic artery, the posterior communicating artery, and finally divides into the anterior cerebral and middle cerebral arteries. Between them these vessels supply most of the cerebral hemispheres: the medial and lateral surfaces of the frontal and parietal lobes, the lateral temporal lobe, and through perforating branches the basal ganglia and internal capsule.

The ophthalmic branch is a clinically important detail, because it makes the retina part of the carotid territory. An embolus from a carotid plaque that happens to enter the ophthalmic artery causes transient monocular visual loss, and that symptom localises disease to the internal carotid on the same side rather than anywhere in the brain.

The posterior circulation: vertebrobasilar

The posterior circulation arises from the vertebral arteries. Their course is worth learning as a sequence, because each step has a clinical consequence.

Subclavian artery Each vertebral artery springs from the first part of the subclavian artery.
Transverse foramina It ascends through the transverse foramina of C6 up to C1.
The atlas turn At C1 it turns sharply over the atlas and pierces the dura.
Foramen magnum It enters the skull through the foramen magnum, not the carotid canal.
Basilar artery The two vertebrals unite at the pontomedullary junction to form the single basilar artery.

On the way, each vertebral artery gives the posterior inferior cerebellar artery and a contribution to the anterior spinal artery, so the vertebral territory is medulla, cerebellum and spinal cord. The basilar then climbs the ventral pons giving perforating branches and the anterior inferior cerebellar and superior cerebellar arteries, and ends by dividing into the two posterior cerebral arteries, which supply the occipital lobe and the inferior and medial temporal lobe. So the posterior circulation covers brainstem, cerebellum and occipital lobe through the posterior inferior cerebellar artery, the anterior inferior cerebellar artery, the superior cerebellar artery and the posterior cerebral arteries.

High yield That sharp turn over the atlas is where the vertebral artery is mechanically vulnerable. Neck hyperextension or forceful rotation can dissect it, and vertebral dissection is a leading cause of posterior circulation stroke in a young patient with no vascular risk factors.

Trap Do not swap the two entry routes. The internal carotid enters through the carotid canal of the petrous temporal bone and feeds the anterior circulation. The vertebral arteries enter through the foramen magnum and feed the posterior circulation. The jugular foramen is an exit, not an entry: it carries the sigmoid sinus out as the internal jugular vein along with cranial nerves IX, X and XI. The foramen ovale transmits the mandibular division of the trigeminal nerve and the accessory meningeal artery, and delivers no vessel to the pons.

Trap The external carotid artery supplies face, scalp and dura, and it feeds no brain tissue. Only its middle meningeal branch enters the skull, and even that supplies dura rather than brain. The middle meningeal artery matters for a different reason entirely: it is the vessel torn in epidural hematoma.

The Circle of Willis: membership is exactly the vessels that link the two inflows

The Circle of Willis is the anastomotic ring on the base of the brain, and its membership is not an arbitrary list. It consists of precisely those vessels that connect the anterior inflow to the posterior inflow, and the two sides to each other. Each internal carotid terminates the anterior inflow and is part of the ring. The two anterior cerebral arteries are the anterior limbs, bridged across the midline by the single anterior communicating artery. Each posterior communicating artery ties that side's internal carotid back to the posterior cerebral artery arising from the basilar, and the two posterior cerebral arteries complete the ring posteriorly.

Vessel	Part of the ring	Relationship to the ring
Internal carotid arteries, both	Yes	Terminate the anterior inflow and form the lateral walls of the ring
Anterior cerebral arteries, both	Yes	Anterior limbs of the ring before running into the interhemispheric fissure
Anterior communicating artery, single	Yes	The one midline bridge between the two anterior cerebral arteries
Posterior communicating arteries, both	Yes	Connect each internal carotid to the posterior cerebral artery on that side
Posterior cerebral arteries, both	Yes	Posterior limbs, arising from the top of the basilar artery
Middle cerebral artery	No	Leaves the ring as a terminal branch of the internal carotid and delivers to the lateral convexity
Basilar artery	No	Sits upstream, feeding the ring by giving the posterior cerebral arteries
Vertebral arteries, both	No	Upstream of the basilar; they feed the ring only indirectly
Middle meningeal artery	No	A branch of the maxillary artery supplying dura, outside the brain's own supply entirely

The functional point of the ring is collateral flow. Because the anterior and posterior inflows are stitched together and the two sides are bridged, occlusion of one feeding vessel proximal to the ring can be compensated: flow crosses the anterior communicating artery from the opposite carotid, or back up a posterior communicating artery from the basilar. This is why a patient can occlude one internal carotid completely and remain asymptomatic if the ring is complete and well developed, and why variant anatomy, which is extremely common, determines how much protection any given person actually has.

Memory hook Count the ring: 2 internal carotid, 2 anterior cerebral, 1 anterior communicating, 2 posterior communicating, 2 posterior cerebral. Nine vessels, one odd one in the middle at the front. The middle cerebral and the basilar deliver to the ring but are not in it.

Rule Anterior equals internal carotid. Posterior equals vertebrobasilar. The communicating arteries are the only structures that belong to both, which is why they define the ring.

High yield The internal carotid contributes to the posterior circulation only indirectly, through the posterior communicating artery. The posterior cerebral artery itself arises from the basilar, so calling the posterior cerebral artery a carotid branch is wrong even though a posterior communicating artery connects them.

Trap A vessel that arises from the ring is not necessarily in it. The middle cerebral artery comes off the internal carotid at the ring and immediately leaves to supply the lateral convexity; it is a delivery vessel, not a ring member. The same logic excludes the ophthalmic artery and the lenticulostriate perforators.

4. Territories and deficits: reading the vessel off the examination

Localisation in cerebrovascular disease works because arterial territories are fixed and the functional map of the brain is fixed. Put the two maps on top of each other and the deficit names the vessel. The discipline is to do it in that order: first ask what surface or structure the artery covers, then ask what that surface does, and let the clinical picture fall out. Memorising artery-deficit pairs without the intervening territory is why these questions get missed.

The three cerebral arteries on the cortical surface

The anterior cerebral artery runs forward and up into the interhemispheric fissure and sweeps over the medial surface of the hemisphere. The homunculus is inverted and draped over the top, so the leg and foot representation sits medially, down in the interhemispheric fissure, exactly where this artery runs. Occlusion therefore gives contralateral leg and foot weakness and sensory loss, with relative sparing of face and arm, and because the medial frontal micturition area is in the same territory, urinary incontinence is characteristic.

The middle cerebral artery fans out across the lateral convexity. It covers the face and arm representation low on the motor and sensory strips, and it covers the language cortex: the Broca area in the inferior frontal gyrus and the Wernicke area in the posterior superior temporal gyrus. Occlusion therefore gives contralateral face and arm weakness and sensory loss with the leg relatively spared, plus aphasia when the dominant hemisphere is involved or hemispatial neglect when the non-dominant hemisphere is involved. Because the territory is large and the vessel is the commonest site for an embolus to lodge, this is the most frequently encountered cortical stroke syndrome.

The posterior cerebral artery takes the occipital lobe and the inferior and medial temporal surface. Occlusion of the main trunk gives contralateral homonymous hemianopia, because the whole primary visual cortex of one hemisphere receives one half-field from both eyes.

Memory hook Medial is leg, lateral is face and arm and language, occipital is vision. Anterior cerebral down the midline, middle cerebral across the side, posterior cerebral around the back.

Trap The homunculus is inverted, and reversing it is the single commonest error here. The leg sits medially in anterior cerebral artery territory and the face sits low on the lateral convexity in middle cerebral artery territory. "Middle cerebral artery for leg, anterior cerebral artery for face" is exactly backwards.

Trap Occipital cortex is posterior cerebral artery territory, and the posterior cerebral artery does not reach the medial frontal and parietal strip. That strip is anterior cerebral artery territory. Swapping the two makes a hemianopia into leg weakness.

Trap None of the three cerebral arteries supplies the cerebellum. The cerebellum is fed by the posterior inferior cerebellar, anterior inferior cerebellar and superior cerebellar arteries off the vertebrobasilar system. An answer choice claiming that the anterior, middle or posterior cerebral artery supplies cerebellum is wrong on its face.

Perforators: small vessels off big trunks

Cortical territories are only half the map. The deep structures are supplied by tiny perforating arteries that come straight off large, high-pressure trunks, and these behave quite differently from cortical branches. Because each perforator is small and arises directly from a major vessel, it is exposed to full systemic pressure, it has no collateral supply, and it can occlude in isolation to produce a small, deep, functionally dense infarct.

The lenticulostriate arteries come off the proximal middle cerebral artery and supply the basal ganglia and internal capsule. An isolated lenticulostriate occlusion gives a pure motor hemiparesis affecting face, arm and leg equally, because the descending fibres are packed tightly together in the internal capsule, and without cortical involvement there is no aphasia, no neglect and no cortical sensory loss.

The pons is supplied the same way. As the basilar artery climbs the ventral surface of the pons it gives paramedian and short circumferential branches that penetrate straight into pontine tissue. The long circumferential branches, which are the anterior inferior cerebellar and superior cerebellar arteries, wrap around the brainstem to reach the cerebellum instead. So a pontine infarct comes from basilar perforators, specifically the paramedian and short circumferential branches, and not from any cerebellar artery and certainly not from a cerebral artery.

The clinical signature of a brainstem perforator infarct is the crossed syndrome: ipsilateral cranial nerve signs together with contralateral limb signs. The cranial nerve nucleus or its exiting fibres are damaged on the same side as the lesion, while the long tracts are damaged above their decussation and therefore produce signs on the opposite side. A patient with an ipsilateral cranial nerve palsy and contralateral hemiparesis has a brainstem lesion, not a hemispheric one.

Rule Small perforators off a big artery are the lacunar vessels of their region. Lenticulostriate off the middle cerebral artery for basal ganglia and internal capsule; paramedian and short circumferential off the basilar for pons. The pattern transfers wherever a large trunk gives tiny penetrating branches.

Rule Long circumferential branches go around to the cerebellum; short penetrating branches go straight in to the brainstem. Name the branch length and you have named the territory.

Trap The posterior inferior cerebellar artery arises from the vertebral artery and supplies lateral medulla and inferior cerebellum, producing Wallenberg syndrome. It does not supply pons. A pontine infarct attributed to the posterior inferior cerebellar artery is a level error: the posterior inferior cerebellar artery works one segment lower, at the medulla.

The artery to territory to deficit table

Artery	Territory	Classic deficit
Anterior cerebral	Medial surface of the hemisphere, including leg and foot cortex and the medial frontal micturition area	Contralateral leg weakness and sensory loss with urinary incontinence; face and arm relatively spared
Middle cerebral, cortical branches	Lateral convexity: face and arm cortex plus Broca and Wernicke language areas	Contralateral face and arm weakness and sensory loss with aphasia in the dominant hemisphere or neglect in the non-dominant hemisphere
Lenticulostriate perforators off the middle cerebral	Basal ganglia and internal capsule	Pure motor hemiparesis of face, arm and leg together, without cortical signs
Posterior cerebral	Occipital lobe with primary visual cortex, inferior and medial temporal surface	Contralateral homonymous hemianopia
Basilar paramedian and short circumferential perforators	Pons	Crossed brainstem syndrome: ipsilateral cranial nerve signs with contralateral limb signs
Posterior inferior cerebellar, off the vertebral	Lateral medulla and inferior cerebellum	Wallenberg syndrome with vertigo, ipsilateral facial sensory loss and contralateral body sensory loss
Anterior inferior cerebellar and superior cerebellar, long circumferential off the basilar	Cerebellum, wrapping around the brainstem to reach it	Ataxia and vertigo on the side of the lesion rather than a pontine crossed syndrome
Ophthalmic, off the internal carotid	Retina	Transient monocular visual loss, localising disease to that internal carotid artery
Anterior spinal, from both vertebrals	Anterior two thirds of the spinal cord: anterior horns, corticospinal and spinothalamic tracts	Bilateral weakness with loss of pain and temperature below the level, vibration and proprioception preserved
Posterior spinal arteries, paired	Posterior third of the cord, essentially the dorsal columns	Loss of vibration and proprioception with motor function and pain and temperature relatively preserved

The spinal cord: a two-territory organ

The spinal cord deserves its own paragraph because its vascular anatomy has a distinctive and highly testable consequence. The anterior spinal artery is a single midline vessel formed from a branch of each vertebral artery, running down the anterior median fissure and reinforced along its length by segmental radicular arteries from the aorta and its branches. It supplies the anterior two thirds of the cord, which contains the anterior horns, the corticospinal tracts and the spinothalamic tracts. The paired posterior spinal arteries supply the posterior third, which is essentially the dorsal columns.

Infarction of the anterior spinal artery therefore produces a dissociated picture: bilateral weakness from anterior horn and corticospinal involvement, bilateral loss of pain and temperature from spinothalamic involvement, and preserved vibration and proprioception because the dorsal columns lie in the posterior third fed by a different vessel. Autonomic function is also lost, giving bladder and bowel dysfunction.

The reinforcement from segmental vessels is unevenly distributed, and the midthoracic cord is the watershed where that reinforcement is thinnest. This is why the midthoracic level is the most vulnerable segment and why anterior spinal artery infarction classically follows aortic surgery, where segmental supply is interrupted and perfusion pressure falls at the same time.

High yield Anterior spinal artery infarction equals motor and pain-temperature loss with dorsal columns spared. The sparing of vibration and proprioception is the diagnostic feature, and it exists purely because the posterior third has its own paired arteries.

Trap The dorsal columns are not anterior spinal artery territory. They occupy the posterior third of the cord and are supplied by the paired posterior spinal arteries, which is exactly why vibration and proprioception survive an anterior spinal artery infarct. Listing dorsal columns under the anterior spinal artery destroys the entire clinical pattern.

Memory hook One artery in front, two behind. The single one takes two thirds and carries motor plus pain and temperature; the pair take one third and carry vibration and position sense.

5. Ischaemia: clot made here, clot made elsewhere, and pressure that never arrives

Arterial supply fails in three mechanically distinct ways, and the distinction is not academic: it determines the onset pattern, the search for a source, and the treatment. A clot can form in place, a clot can arrive from somewhere else, or perfusion pressure can simply fall below what the most distal tissue needs. All three end in infarction of brain that has no fuel reserve, so all three are emergencies, but they are investigated completely differently.

Thrombosis: the clot is made where it lodges

A thrombus forms in place, on a diseased vessel wall. The classic sites are an atherosclerotic plaque at the carotid bifurcation, where turbulent flow and endothelial injury coincide, and the small penetrating arteries, where chronic hypertension produces lipohyalinosis and progressive luminal narrowing. Because the lumen closes progressively rather than all at once, and because platelet aggregation on a plaque waxes and wanes, the symptoms often build in a stepwise or stuttering way, with partial deficits that fluctuate before becoming fixed.

A thrombotic occlusion of a small penetrating artery is the substrate of lacunar infarction, and the risk factor profile is the one that damages small vessels: long-standing hypertension above all, with diabetes and smoking alongside it. A thrombotic occlusion of a large extracranial vessel such as the internal carotid may be tolerated if the Circle of Willis is complete, which is one more reason the ring matters.

Embolism: the clot is made elsewhere and travels

An embolus forms somewhere upstream and then travels downstream until the lumen narrows enough to trap it. The dominant cardiac source is the left atrium in atrial fibrillation, where stasis in a fibrillating atrium and its appendage lets thrombus form and then release. Other sources are a proximal atherosclerotic plaque, typically at the carotid bifurcation, and valvular material from endocarditis or a prosthetic valve.

Because the vessel is open one moment and completely blocked the next, an embolic deficit is abrupt and maximal at onset. The commonest destination is the middle cerebral artery, both because it carries the largest share of carotid flow and because it continues in nearly the same direction as the parent vessel, so a travelling particle tends to be carried into it.

Feature	Thrombosis	Embolism
Where the clot forms	In place, on a diseased wall	Elsewhere, then travels and lodges
Typical source	Atherosclerotic plaque at the carotid bifurcation, or a small penetrating artery	Left atrium in atrial fibrillation, a proximal plaque, or a valve
Onset pattern	Stepwise or stuttering as the lumen progressively closes	Abrupt, maximal at onset
Classic site of occlusion	Carotid bifurcation, or deep perforators giving lacunes	Middle cerebral artery

Memory hook Thrombus: made here, builds up. Embolus: made elsewhere, lands all at once. Both end in a blocked artery and an infarcted territory.

Trap Travelling is the defining feature of an embolus. A thrombus stays where it formed. Describing a thrombus as travelling inverts the only distinction between the two terms.

Trap Emboli are not a venous-only phenomenon. They arise in veins and arteries alike, and arterial emboli from the heart or the carotid are the usual cause of embolic brain infarction. Restricting embolism to veins would make cardioembolic stroke impossible.

Trap Neither thrombosis nor embolism involves rupture of the vessel wall. Rupture produces haemorrhagic stroke, which is a separate category with a separate management pathway. Occlusion by clot and rupture of the wall are the two top-level branches of cerebrovascular disease, and conflating them leads to giving a thrombolytic to a patient who is bleeding.

Watershed infarction: the end of two supply lines at once

A watershed zone is the border region where the most distal twigs of two adjacent arterial territories meet. There are two classic cortical watersheds: the strip between anterior cerebral and middle cerebral territories along the frontal and parietal convexity, and the strip behind it between middle cerebral and posterior cerebral territories. There is also the midthoracic spinal cord watershed described earlier.

The vulnerability is purely hydraulic. A watershed sits at the very end of both supply lines, so perfusion pressure there is the lowest anywhere in the territory of either vessel. When pressure falls globally, as in severe systemic hypotension, cardiac arrest, or severe carotid stenosis that drops pressure distally, the whole brain loses pressure but only the watersheds fall below the threshold for viability. The result is bilateral infarction confined to those border strips.

Clinically that produces the man-in-a-barrel pattern: bilateral proximal arm and leg weakness, because the shoulder and hip representations sit in the anterior-to-middle cerebral border zone, while the distal hand and the face, which are represented more laterally and centrally within the middle cerebral territory, are spared. The key recognition point is that the deficit is bilateral and does not match any single named artery syndrome, which tells you the problem was systemic perfusion rather than one occluded vessel.

High yield A bilateral, symmetrical deficit after cardiac arrest or profound hypotension is watershed infarction, not two simultaneous strokes. Global hypotension picks off the end zones first, which is why the pattern is proximal and bilateral.

Rule A named artery syndrome points to an occlusion in that artery. A bilateral border-zone syndrome points to a pressure failure upstream of everything. Pattern first, then mechanism.

Trap A watershed is the opposite of the Circle of Willis, and the two are easy to confuse because both are about collateral supply. The ring at the base is a large anastomosis that protects against proximal occlusion. A watershed is a distal end zone where two territories just barely meet, and it is the most vulnerable tissue in the brain rather than the best protected.

Trap Do not reach for venous or secretory structures as the answer to a perfusion question. Dural venous sinuses are endothelium-lined channels between the dural layers carrying blood out of the skull toward the internal jugular vein, and the choroid plexus is the cerebrospinal fluid secreting tissue inside the ventricles, richly perfused by choroidal arteries. Neither is a border zone between arterial territories.

6. Rupture and malformation: haemorrhage by compartment, aneurysm, and arteriovenous shunts

When a vessel wall fails rather than occludes, the first question is not which artery but which space the blood entered. Each anatomical compartment has its own usual source vessel, its own tempo, its own imaging appearance and its own clinical signature. Working from the skull inward gives the list in a reliable order, and the compartment names the bleed.

Epidural Between the skull and the dura, outside everything.
Subdural Between the dura and the arachnoid.
Subarachnoid Between the arachnoid and the pia, mixing with cerebrospinal fluid.
Intraparenchymal Within the substance of the brain itself.
Intraventricular Inside the ventricular system, in the cerebrospinal fluid pathway.

The haemorrhage compartment comparison

Bleed	Space and usual source	Typical setting	Clinical signature
Epidural hematoma	Between skull and dura; arterial, classically the middle meningeal artery, a branch of the maxillary artery	Head trauma, often with a temporal bone fracture	Lucid interval after the injury, then rapid deterioration as arterial blood accumulates under pressure
Subdural hematoma	Between dura and arachnoid; venous, from torn bridging veins crossing to the sinuses	The elderly, and others with brain atrophy that stretches the bridging veins; often minor trauma	Slower, fluctuating course with confusion and headache, sometimes with no remembered injury
Subarachnoid hemorrhage	Between arachnoid and pia; arterial blood mixing directly with cerebrospinal fluid, classically a ruptured berry aneurysm	Spontaneous, often during exertion; also an arteriovenous malformation in a younger patient	Thunderclap worst-headache-of-life, meningism, and blood in the cerebrospinal fluid at lumbar puncture
Intraparenchymal hemorrhage	Within brain tissue; typically hypertensive rupture of lenticulostriate vessels	Chronic hypertension, with Charcot-Bouchard microaneurysms on small penetrating arterioles	Focal deficit that progresses over minutes to hours as the hematoma expands, with headache and vomiting
Intraventricular hemorrhage	Inside the ventricles; extension from an adjacent parenchymal bleed, or primary in the premature neonate	Deep hypertensive bleeds that reach a ventricle; germinal matrix bleeding in prematurity	Blood in the cerebrospinal fluid pathway can block flow and absorption, so acute hydrocephalus follows

Memory hook Order by layer from outside in: epidural, subdural, subarachnoid, intraparenchymal, intraventricular. Arterial outside the dura, venous just inside it, arterial again in the subarachnoid space.

Trap Only bleeds into the meningeal and brain compartments count as intracranial hemorrhage types. Intracochlear hemorrhage is bleeding into the fluid spaces of the inner ear within the temporal bone, outside the meningeal compartments altogether, and it presents with sudden hearing loss and vertigo rather than with headache, focal deficit or raised intracranial pressure.

High yield Match the vessel type to the compartment and the tempo follows. Epidural is arterial and fast with a lucid interval; subdural is venous and slow with a fluctuating course; subarachnoid is arterial into cerebrospinal fluid and instantaneous, giving the thunderclap headache. Intraventricular blood is the one that causes hydrocephalus, because it is in the cerebrospinal fluid pathway.

Saccular aneurysm: a weak apex at a branch point

Saccular or berry aneurysms form at arterial bifurcations, and above all at the bifurcations of the Circle of Willis. The three classic sites are the anterior communicating artery, the posterior communicating artery and the middle cerebral trifurcation. The reason is mechanical. At a branch point the muscular media thins, and the incoming column of blood strikes the apex of the bifurcation directly, so wall shear stress is concentrated at exactly the spot where the wall is weakest. Add chronic hypertension and the apex balloons outward over years into a dome sitting on a narrow neck.

That neck is the therapeutic handle. Because a berry aneurysm is a sac on a stalk rather than a diffuse dilation, a surgeon can place a clip across the stalk and exclude the sac from the circulation, or an interventionalist can pack the dome with coils through the neck. Rupture spills arterial blood into the subarachnoid space and produces the thunderclap worst-headache-of-life, with meningism from blood irritating the meninges.

A posterior communicating artery aneurysm has an additional localising sign worth remembering, because the oculomotor nerve runs immediately alongside it: enlargement compresses the nerve and produces a third nerve palsy, with the pupil involved because the parasympathetic fibres sit superficially.

Trap Not every arterial dilation is a clippable berry aneurysm. A fusiform aneurysm is a dilation of a whole vessel segment, usually atherosclerotic, and it has no narrow neck to clip. And a Charcot-Bouchard microaneurysm on a small penetrating arteriole in a chronically hypertensive patient is microscopic: it bleeds into deep brain tissue as an intraparenchymal hemorrhage and is far too small to clip. Mid-vessel and stented describes neither.

Trap Veins and capillaries do not form saccular aneurysms. Veins are low-pressure, thin-walled channels whose pathology is thrombosis or varicosity. Capillaries are a single endothelial layer with no media to balloon. Saccular aneurysm is an arterial branch-point lesion, and pressure is why.

High yield Berry aneurysm at a bifurcation on the Circle of Willis; rupture means subarachnoid hemorrhage and thunderclap headache; the narrow neck is what allows clipping at the stalk or coiling of the dome.

Arteriovenous malformation: arterial pressure on a venous wall

An arteriovenous malformation is a congenital tangle of vessels, called the nidus, in which arteries shunt straight into veins with no intervening capillary bed. Remove the capillary bed and two things go wrong simultaneously. First, arterial pressure is delivered directly onto thin-walled veins, which dilate, become tortuous and may rupture. Second, the low-resistance shunt steals flow that should have perfused the surrounding cortex, so adjacent brain is chronically underperfused.

The clinical picture follows from those two mechanisms. Cortical irritation by the abnormal vessels causes seizures, often the presenting event in a young patient. Headache is common. Progressive focal deficit develops from the steal effect. And the dilated veins can rupture, producing intraparenchymal or subarachnoid hemorrhage in a patient far too young for hypertensive or atherosclerotic disease. Treatment eliminates the nidus, by surgical resection, endovascular embolization, or stereotactic radiosurgery.

Dural arteriovenous fistula: the same physics, in the dura

A dural arteriovenous fistula is a direct connection from meningeal arteries into a dural venous sinus or a cortical vein. Unlike an arteriovenous malformation it is usually acquired rather than congenital, developing after sinus thrombosis, trauma or surgery. Because an arterial pressure head is now feeding a venous channel, flow through the sinus becomes turbulent and venous drainage out of the skull is impeded.

The giveaway symptom is pulse-synchronous tinnitus: a whooshing bruit heard in one ear and timed exactly to the heartbeat, because the patient is hearing turbulent arterial-pressure flow in a sinus close to the ear. The back pressure from impeded venous drainage can raise intracranial pressure, and the pressurised venous channels can also hemorrhage.

Rule Arterial pressure delivered into a vein always does the same two things: it makes noise, and it raises venous pressure. Hold that rule and both the arteriovenous malformation and the dural fistula become predictable rather than memorised.

Lesion	What connects to what	Hallmark presentation
Arteriovenous malformation	Congenital nidus: cerebral arteries shunt into veins with no capillary bed	Seizures, headache, progressive deficit from steal, and hemorrhage in a young patient
Dural arteriovenous fistula	Acquired: meningeal arteries connect straight into a dural sinus or cortical vein	Pulse-synchronous whooshing tinnitus in one ear, raised intracranial pressure, possible hemorrhage
Saccular aneurysm	One focal outpouching of an arterial wall at a bifurcation, with no shunt at all	Thunderclap headache from subarachnoid hemorrhage when the dome ruptures
Cerebral venous sinus thrombosis	A clot inside a dural sinus, blocking outflow	Headache and raised intracranial pressure from obstructed venous drainage

Trap Keep the four vascular lesions above distinct. A clot in a dural sinus is cerebral venous sinus thrombosis, which obstructs outflow and raises intracranial pressure, not a shunt. A ruptured aneurysm is one focal outpouching bursting, usually into the subarachnoid space, not a sustained artery-to-vein connection. And an arteriovenous malformation is a vascular anomaly with no neoplastic growth, so calling it a benign tumor is wrong: a tumor is a mass of proliferating cells.

Trap A middle cerebral artery aneurysm, an obstructed cerebral aqueduct and a choroid plexus papilloma are three unrelated things that get offered together. The aneurysm is a focal bulge of an arterial wall at a bifurcation threatening subarachnoid hemorrhage. Aqueductal stenosis obstructs cerebrospinal fluid between the third and fourth ventricles and causes obstructive hydrocephalus. A choroid plexus papilloma oversecretes cerebrospinal fluid and can obstruct flow, producing hydrocephalus. None of them produces a bruit.

High yield Seizure plus hemorrhage in a young patient with no vascular risk factors points to an arteriovenous malformation. Pulse-synchronous unilateral whooshing points to a dural arteriovenous fistula. Thunderclap headache points to a ruptured berry aneurysm. Three vascular lesions, three distinct opening lines.

Memory hook No capillary bed means arterial pressure lands on veins. Inside the brain that gives seizures then a bleed; inside the dura it gives a noise in the ear then raised pressure.

7. Venous drainage: valveless channels in the dura, and the path to the jugular

Venous outflow from the brain does not use ordinary veins for most of its course. It uses dural venous sinuses, which are structurally unlike any vein elsewhere in the body, and that structural difference has direct clinical consequences. Everything eventually converges on the internal jugular vein, but there are two separate routes to it, and one of them bypasses the main confluence entirely.

What a dural sinus actually is

A dural venous sinus is an endothelium-lined channel running between the periosteal and meningeal layers of the dura. Its walls are therefore tough dura, not a vein wall. It has no smooth muscle media, so it cannot constrict or dilate actively, and it cannot collapse under external pressure the way a thin-walled vein does. Most importantly, it carries no valves.

No valves means direction of flow is set purely by the pressure gradient across the channel. In normal circumstances that gradient carries blood from the brain toward the internal jugular vein, but if the gradient reverses, so does the flow. This is not a theoretical point. Infection from the face or scalp can track inward along emissary veins through the skull, or backwards along the ophthalmic veins from the orbit, and reach the cavernous sinus. That is the anatomical route to cavernous sinus thrombosis, and it is the reason a seemingly trivial facial infection in the territory draining to the ophthalmic veins is treated seriously.

The sinuses have two intakes. They receive cerebral veins, superficial and deep, carrying blood out of the brain. And through arachnoid granulations they receive cerebrospinal fluid. Then they funnel everything to the internal jugular vein.

Feature	Dural venous sinus	Ordinary systemic vein
Wall	Endothelium lining a space between the two dural layers; the wall is dura	Endothelium with a smooth muscle media and adventitia
Valves	None, so flow direction follows the pressure gradient in either direction	Present, enforcing one-way flow back toward the heart
Function	Pure draining channel that also absorbs cerebrospinal fluid through arachnoid granulations	Capacitance and return of blood to the heart

Trap Dural sinuses are neither arteries nor lymphatics. Arteries carry blood away from the heart in thick elastic and muscular walls, while sinuses are purely draining channels. Lymphatics return interstitial fluid through nodes, and the brain's equivalent clearance uses perivascular glymphatic and meningeal lymphatic routes rather than the dural sinuses themselves.

High yield Dural walls and no valves. Those two features together explain why facial and orbital infection can spread backwards into the cavernous sinus, and why raised central venous pressure is transmitted straight back into the cranium.

The deep venous system, step by step

Deep venous drainage follows a consistent geometry: inward to the midline, backward along the tentorium, then laterally out of the skull. Each step is a confluence where named tributaries join, which is what makes the sequence learnable.

Deep tributaries Veins of the deep white matter, thalamus and basal ganglia collect toward the midline.
Internal cerebral veins The paired internal cerebral veins receive that deep drainage and run backward on the roof of the third ventricle.
Great cerebral vein of Galen The two internal cerebral veins unite beneath the splenium of the corpus callosum as the single great cerebral vein, where the inferior sagittal sinus also joins.
Straight sinus That single trunk continues backward within the tentorium as the straight sinus.
Confluence of sinuses At the internal occipital protuberance the straight sinus meets flow coming back along the superior sagittal sinus and the two mix.
Transverse sinus Flow turns laterally in the attached margin of the tentorium.
Sigmoid sinus The transverse sinus curves as the S-shaped sigmoid sinus toward the skull base.
Internal jugular vein The sigmoid sinus exits the jugular foramen and becomes the internal jugular vein.

The superficial system is simpler: cortical veins on the convexity drain upward into the superior sagittal sinus, which runs front to back along the upper midline in the attached margin of the falx, collecting as it goes, and delivers to the confluence. So superficial cortex and deep grey matter arrive at the confluence by two different paths and leave together.

Memory hook Deep brain to internal cerebral to great cerebral vein to straight sinus to confluence to transverse to sigmoid to internal jugular. Midline, then back, then sideways, then out.

Trap The straight sinus and the superior sagittal sinus are not interchangeable. The straight sinus carries deep venous blood from the great cerebral vein and the inferior sagittal sinus backward to the confluence, and it bears no arachnoid granulations. The superior sagittal sinus runs in the upper midline collecting superficial cortical veins and absorbing cerebrospinal fluid, draining front to back toward the same confluence.

Arachnoid granulations project into one sinus only

Arachnoid granulations are the macroscopic form of arachnoid villi: tufts of arachnoid membrane that push through the dura and protrude into the superior sagittal sinus and its lateral lacunae along the top of the midline. They exist nowhere else in any quantity, and that single anatomical fact makes the superior sagittal sinus the site of cerebrospinal fluid absorption.

Absorption is driven by a pressure gradient rather than by active transport. Normal cerebrospinal fluid pressure of 70 to 180 mm of water exceeds the venous pressure inside the sinus, and that difference drives bulk, one-way movement of cerebrospinal fluid, carrying its dissolved protein with it, from the subarachnoid space into venous blood. The granulations behave as pressure-sensitive one-way valves: they open when cerebrospinal fluid pressure exceeds sinus pressure and close when it does not, so blood never flows backward into the subarachnoid space.

Two consequences follow immediately. If the granulations are plugged or absent, absorption fails and pressure rises throughout the whole cerebrospinal fluid system, which is communicating hydrocephalus. And if sinus venous pressure rises, for example because a sinus is thrombosed or a dural fistula has pressurised it, the absorption gradient shrinks and intracranial pressure climbs even though the granulations themselves are normal.

Rule Absorption needs cerebrospinal fluid pressure to exceed sinus pressure. Break the gradient from either side, by plugging the villi or by raising sinus pressure, and intracranial pressure rises.

Trap Arachnoid granulations do not project into the straight, sigmoid or cavernous sinus. The sigmoid sinus is simply the S-shaped continuation of the transverse sinus that leaves the jugular foramen as the internal jugular vein. The cavernous sinus flanks the pituitary and the sella and transmits cranial nerves III, IV, V1, V2 and VI plus the internal carotid artery. Neither absorbs cerebrospinal fluid.

The petrosal sinuses: a second route to the jugular

The cavernous sinus has its own outflow, and the important structural point is that neither of its drainage channels passes through the confluence of sinuses. Both petrosal sinuses run along the petrous ridge draining the cavernous sinus, and they take slightly different paths to the same destination.

Sinus	Receives from	Drains to
Superior sagittal	Superficial cortical veins and cerebrospinal fluid through arachnoid granulations	Confluence of sinuses, front to back
Inferior sagittal	Medial hemispheric surface	Joins the great cerebral vein to form the straight sinus
Straight	Great cerebral vein of Galen and the inferior sagittal sinus	Confluence of sinuses
Transverse	Confluence of sinuses, plus the superior petrosal sinus	Sigmoid sinus on the same side
Sigmoid	Transverse sinus	Internal jugular vein through the jugular foramen
Cavernous	Ophthalmic veins and facial tributaries; transmits cranial nerves III, IV, V1, V2, VI and the internal carotid artery	Superior and inferior petrosal sinuses
Superior petrosal	Cavernous sinus	Transverse sinus, then sigmoid, then internal jugular vein
Inferior petrosal	Cavernous sinus	Straight into the internal jugular vein at the jugular foramen

So the inferior petrosal sinus empties directly into the internal jugular vein at the jugular foramen, and the superior petrosal sinus joins the transverse sinus, which becomes the sigmoid sinus and then the same internal jugular vein. Either way the cavernous sinus reaches the jugular without ever passing through the superior sagittal or straight sinus route. That separate path is also the anatomical explanation for how facial and orbital infection tracks back into the cavernous sinus: the connection is direct and valveless, with no interposed confluence.

High yield Petrosal sinuses drain the cavernous sinus to the jugular system and bypass the confluence entirely. This is why cavernous sinus pathology presents with cranial nerve III, IV, V1, V2 and VI findings and orbital congestion rather than with the raised-pressure picture of a blocked superior sagittal sinus.

Trap Do not route the petrosal sinuses through the confluence. The confluence of sinuses, behind the internal occipital protuberance, is where superior sagittal and straight sinus flow meets before splitting into the transverse sinuses. The cavernous sinus and its petrosal outflow are a separate circuit on the skull base.

Memory hook Inferior petrosal goes down and straight out to the jugular. Superior petrosal goes back and joins the transverse. Both end at the internal jugular vein; neither visits the confluence.

8. Cerebrospinal fluid: secreted, circulated, absorbed, and read at the needle

Cerebrospinal fluid is not an ultrafiltrate of plasma that happens to sit around the brain. It is an actively secreted fluid whose composition is set by an epithelium, moved along a one-way path by a pressure gradient, and returned to the blood at a single anatomical site. Every clinical problem in this area is a failure at one of those three stages, so learn the system as make, move, absorb.

Made: the choroid plexus epithelium is the blood-cerebrospinal fluid barrier

Cerebrospinal fluid is secreted by the choroid plexus, which is a frond of fenestrated capillaries covered by a layer of specialized cuboidal ependymal cells joined to each other by tight junctions. Note the architecture carefully, because it is the mirror image of the blood-brain barrier: here the capillary is leaky and the epithelium on top of it is sealed. Those tight-junctioned ependymal cells are the blood-cerebrospinal fluid barrier.

Secretion is active, not filtrative. The ependymal cells pump sodium, chloride and bicarbonate into the ventricular space, and water follows the osmotic gradient. Carbonic anhydrase supplies the bicarbonate and the protons needed for that ion transport, which gives a direct pharmacological lever: acetazolamide inhibits carbonic anhydrase and reduces cerebrospinal fluid production. That is why acetazolamide is used in conditions of raised cerebrospinal fluid pressure.

Choroid plexus sits in the floor of both lateral ventricles and in the roof of the third and fourth ventricles. Because the largest mass of it is in the lateral ventricles, secretion begins upstream of the entire flow path, which is why a block anywhere downstream causes upstream dilation.

Trap Keep the two ends of the system straight. Arachnoid granulations are the absorption end, returning cerebrospinal fluid into the superior sagittal sinus. Dural sinuses are the venous channels that receive cerebrospinal fluid once it has been absorbed, not a source of it. And astrocytes form the end-feet supporting the blood-brain barrier and buffer extracellular potassium, but they do not secrete cerebrospinal fluid. Only the specialized ependymal cells of the choroid plexus do.

High yield Carbonic anhydrase drives choroid plexus secretion, so acetazolamide turns cerebrospinal fluid production down. That single link converts an enzyme fact into a treatment.

The numbers, and why they matter

Total cerebrospinal fluid volume is about 150 mL, of which roughly 25 mL sits inside the ventricles and the remainder in the subarachnoid space and basal cisterns. The choroid plexus secretes about 400 to 500 mL per day. Put those two numbers together and the dynamic becomes obvious: production replaces the entire volume around three times every day.

That turnover rate is the reason absorption must match production exactly, and the reason any outflow block raises pressure within hours rather than over weeks. A system that replaces its whole contents three times a day cannot tolerate even a modest mismatch between inflow and outflow for long, because the container is rigid.

Normal opening pressure at lumbar puncture, measured with the patient in the lateral decubitus position, is 70 to 180 mm of water. That is the reference figure you compare against in hydrocephalus and in idiopathic intracranial hypertension, and it is also the pressure that drives absorption across the arachnoid granulations into the superior sagittal sinus.

Parameter	Normal value	Why it matters
Total volume	About 150 mL, with about 25 mL in the ventricles	A small, rigid-container volume, so pressure rises quickly when outflow fails
Production rate	About 400 to 500 mL per day	Roughly three complete turnovers daily, so absorption must match production exactly
Opening pressure	70 to 180 mm of water in lateral decubitus	Exceeds sinus venous pressure, which is what drives one-way absorption

Memory hook 150 made, 450 a day, 3 turnovers. Pressure 70 to 180 mm of water, measured lying on the side.

Trap Watch the units and the orders of magnitude together. A litre of cerebrospinal fluid with trivial production and near-zero pressure would describe a flaccid, barely circulating compartment, which is not what this system is. Fifty millilitres at 300 mm of water describes a small and dangerously hypertensive compartment, since 300 mm of water is already a pathological pressure. Five litres a day is cardiac-output scale, far beyond what choroid plexus secretion can produce, and quoting the pressure in millimetres of mercury borrows the unit used for arterial rather than routine cerebrospinal fluid measurement.

Moved: one path, one direction, driven by pressure

Cerebrospinal fluid moves down a pressure gradient from where it is secreted to where it is absorbed, so the circulation route is simply the anatomy of that one-way path. Secretion is highest upstream and absorption happens at the far end, which fixes the direction.

Lateral ventricles Choroid plexus in the floor of each lateral ventricle secretes first and in the greatest volume.
Interventricular foramina of Monro Paired channels carrying flow from each lateral ventricle into the midline third ventricle.
Third ventricle A narrow midline slit between the thalami, with its own choroid plexus in the roof.
Cerebral aqueduct The narrowest point of the entire pathway, running through the midbrain.
Fourth ventricle Between pons and cerebellum, with choroid plexus in its roof.
Lateral and median apertures Flow leaves the ventricular system through the two lateral apertures of Luschka and the median aperture of Magendie.
Subarachnoid space Fluid circulates over the convexity and around the cord and spinal roots, cushioning the whole central nervous system.
Arachnoid granulations Tufts projecting into the superior sagittal sinus act as pressure-sensitive one-way valves.
Superior sagittal sinus Cerebrospinal fluid with its dissolved protein crosses into venous blood and leaves toward the internal jugular vein.

Rule The level of a block names the hydrocephalus. Learn the pathway in order and every obstructive pattern can be derived rather than memorised: whatever lies upstream of the block dilates, and whatever lies downstream does not.

High yield The cerebral aqueduct is the narrowest segment of the pathway, which is why aqueductal stenosis is the classic cause of obstructive hydrocephalus and why it dilates the lateral and third ventricles while leaving the fourth normal.

Composition: actively secreted, therefore deliberately different from plasma

Because the choroid plexus epithelium secretes rather than filters, it gets to choose the composition of the fluid, and what it chooses is a stable environment for neurons. Compared with plasma, cerebrospinal fluid holds far less protein, about 34 mg/dL against roughly 6500 mg/dL. It holds almost no cells, fewer than five lymphocytes per microliter. Potassium and calcium are lower than in plasma. Sodium and osmolality match plasma closely, and chloride runs slightly higher.

The low potassium is not incidental. Extracellular potassium concentration sets the resting membrane potential and therefore neuronal excitability, so keeping cerebrospinal fluid potassium low and stable protects the brain from the swings in plasma potassium that occur with diet, exercise and acid-base shifts. Low calcium similarly stabilises synaptic transmission and excitability.

Constituent	Cerebrospinal fluid compared with plasma	Significance
Protein	Much lower: about 34 mg/dL against roughly 6500 mg/dL	Any rise means the barrier has been breached, by infection, obstruction or inflammation
Cells	Almost none: fewer than five lymphocytes per microliter	A raised count is pleocytosis, the hallmark of meningitis or other inflammation
Potassium and calcium	Both lower than plasma	Stabilises neuronal excitability and synaptic transmission against plasma swings
Sodium and osmolality	Essentially equal to plasma	No osmotic gradient, so water does not shift between blood and cerebrospinal fluid
Chloride	Slightly higher than plasma	Reflects active chloride secretion by the choroid plexus epithelium

Trap Cerebrospinal fluid is not a plasma filtrate and its composition is not identical to plasma. Identical composition would imply passive filtration, but active transport deliberately makes it differ. Likewise, raised protein and raised cell counts are abnormal, not normal: raised protein points to infection, blocked flow, or inflammatory breakdown of the barrier, and a raised cell count points to inflammation.

Reading a lumbar puncture

Interpretation is systematic. Ask which cell type dominates, then what happened to glucose and protein, then whether the fluid looks turbid or bloody. Those three questions separate the major diagnoses.

Appearance and cells	Glucose and protein	Diagnosis
Cloudy and turbid, with thousands of neutrophils	Glucose low, protein high	Bacterial meningitis; treat empirically at once
Clear, with a lymphocyte predominance	Glucose normal, protein mildly raised	Viral meningitis
Clear, with a mildly raised lymphocyte count and oligoclonal IgG bands	Glucose normal	Active multiple sclerosis
Uniformly bloody, or xanthochromic, with red cells that do not clear across successive tubes	Protein raised from blood breakdown products	Subarachnoid hemorrhage
Malignant cells on cytology, lymphocytic rather than neutrophilic	Glucose markedly low, protein high	Meningeal metastasis, or carcinomatous meningitis

The logic behind the bacterial pattern is worth stating because it makes the numbers predictable. Bacteria recruit neutrophils in enormous numbers, which makes the fluid turbid; both the organisms and the infiltrating leukocytes consume glucose, which drives the glucose down; and the inflamed, breached barrier lets plasma protein in, which drives the protein up. A virus recruits lymphocytes and does not consume glucose, so glucose stays normal. That is the whole differential in one mechanism.

High yield Neutrophils plus low glucose equals bacterial meningitis. Lymphocytes plus oligoclonal IgG bands equals multiple sclerosis. Red cells that do not clear across successive tubes, plus xanthochromia, equals subarachnoid hemorrhage. Three patterns, three answers.

Trap Do not let a lymphocytic picture default to viral. Active multiple sclerosis gives a mildly raised lymphocyte count with oligoclonal IgG bands and normal glucose, not a neutrophil flood. Carcinomatous meningitis also gives a lymphocytic rather than neutrophilic picture, but with malignant cells, high protein and markedly low glucose. The glucose and the cytology separate them.

Trap Bloody fluid has two explanations and they are distinguished by a single bedside observation. In a traumatic tap the red cell count falls across successive tubes as the puncture-site blood washes out. In subarachnoid hemorrhage the red cells do not clear, and xanthochromia appears from broken-down hemoglobin.

9. Hydrocephalus: find the level, name the type, and understand the shunt

Hydrocephalus is always the same underlying problem: a mismatch between cerebrospinal fluid production and cerebrospinal fluid outflow, in a rigid container. What differs between types is where the mismatch is created. So the diagnostic move is anatomical, not descriptive: find the level of the block relative to the subarachnoid space, and the type names itself.

Obstructive versus communicating

Obstructive hydrocephalus, also called non-communicating hydrocephalus, blocks flow inside the ventricular system. The three narrow points are the interventricular foramen of Monro, the cerebral aqueduct, and the outlets of the fourth ventricle. Because the block is proximal to the subarachnoid space, cerebrospinal fluid never reaches it, and only the ventricles upstream of the block dilate. That upstream-only pattern is diagnostically useful: aqueductal stenosis dilates the lateral and third ventricles and leaves the fourth normal, while a mass at one foramen of Monro dilates only that lateral ventricle.

Communicating hydrocephalus leaves the ventricles freely connected to each other and to the subarachnoid space, so flow through the whole pathway is intact. The failure is at the end of the line: absorption through the arachnoid villi into the venous sinuses. Since the obstruction is downstream of everything, pressure rises throughout the entire system, and the ventricles and the subarachnoid space enlarge together.

Type	Where the block sits	What dilates and what the pressure does
Obstructive, also called non-communicating	Inside the ventricular system: a foramen of Monro, the cerebral aqueduct, or the fourth ventricular outlets	Only ventricles upstream of the block dilate; pressure rises, often acutely
Communicating	At the arachnoid villi, where absorption into the venous sinuses fails	The whole system including the subarachnoid space enlarges; pressure rises throughout
Normal pressure hydrocephalus	A chronic communicating failure in older adults	Ventricles enlarge slowly, and measured opening pressure sits in the normal range
Overproduction	At the choroid plexus itself, as with a papilloma	Uncommon mechanism; the lesion can also obstruct flow mechanically

Rule Block inside the ventricles equals obstructive, with upstream dilation only. Block at the villi equals communicating, with everything dilating. The relationship of the block to the subarachnoid space is the only thing the two words describe.

Trap The two labels are not interchangeable, and reversing them is the standard error. Communicating means cerebrospinal fluid still reaches the subarachnoid space, which is exactly what an intraventricular obstruction prevents. If fluid cannot get out of the ventricles, the system by definition does not communicate.

Causes of communicating hydrocephalus all sit at the villi

Because communicating hydrocephalus is defined by absorptive failure, every cause of it is a failure at the arachnoid villi. There are three ways that membrane stops working.

Congenital absence of arachnoid villi The absorptive membrane was never formed, so production has nowhere to go from birth.
Red cells and breakdown products plugging the villi After subarachnoid or intraventricular hemorrhage, erythrocytes and their degradation products physically obstruct the villi. This is why hydrocephalus is a recognised complication of a subarachnoid bleed days after the event.
Protein and exudate after infection Meningitis leaves the villi scarred and coated with protein and inflammatory exudate, so absorption is permanently impaired.

Pressure then rises throughout the whole system rather than only upstream of one narrow point, which is the imaging signature: everything is big, including the subarachnoid space over the convexity.

Trap Narrow points are obstructive, not communicating. Narrowing of the cerebral aqueduct blocks flow between the third and fourth ventricles and dilates the lateral and third ventricles only. A mass at the interventricular foramen, such as a colloid cyst, blocks one lateral ventricle at its outlet. Both are obstructive patterns and neither belongs on a list of communicating causes.

Trap A choroid plexus lesion such as a papilloma overproduces cerebrospinal fluid, which is an uncommon mechanism distinct from either obstruction pattern. Do not file it under communicating hydrocephalus just because pressure rises everywhere: the failure is at production, not at absorption.

High yield Blood and infection are the two acquired causes of communicating hydrocephalus, and both act by plugging or scarring the same membrane. Hydrocephalus appearing days to weeks after a subarachnoid hemorrhage or after meningitis is this mechanism.

Normal pressure hydrocephalus

Normal pressure hydrocephalus is a chronic communicating hydrocephalus of older adults. It is grouped separately because of a measurement paradox: the ventricles enlarge so slowly that the compliance of the brain and the compensatory mechanisms keep up, and by the time the patient presents the measured opening pressure sits within the normal range. The pressure was never normal during the enlargement; it is normal at the moment you measure it.

The clinical picture is a direct mechanical consequence of ventricular enlargement. Dilated ventricles stretch the periventricular white matter, and the fibres stretched hardest are the ones curving closest around the ventricle. Those are the long corticospinal fibres from the medial leg representation and the bladder-control fibres from the medial micturition cortex, both of which arise on the medial surface and must swing around the lateral ventricle to descend. Stretching them gives a triad.

Element of the triad	Mechanism	Clinical form
Gait problems	Stretch of medial leg corticospinal fibres curving around the ventricle	A magnetic, shuffling, wide-based gait; appears first and responds best to shunting
Urinary incontinence	Stretch of bladder-control fibres from the medial micturition cortex	Urgency progressing to frank incontinence
Dementia	Diffuse periventricular white matter stretch and disconnection	Apathetic, slowed, subcortical cognitive decline

Memory hook Wet, wacky and wobbly in an older adult with large ventricles and normal measured pressure. The medial leg fibres stretch first, so wobbly comes first too.

Trap The normal pressure hydrocephalus triad is not the triad of acutely raised intracranial pressure. Headache, vomiting and papilledema form that other triad, seen in obstructive hydrocephalus or a mass lesion. Vertigo, tinnitus and hearing loss are inner ear and vestibulocochlear nerve symptoms, as in Meniere disease or a vestibular schwannoma. Fever with nuchal rigidity and a petechial rash suggests meningitis, particularly meningococcal. Four different triads, and only one of them is wet, wacky and wobbly.

Trap Do not generalise the normal pressure label. Most hydrocephalus runs at raised pressure. Normal pressure hydrocephalus is one specific chronic communicating form whose measured pressure happens to sit in the normal range, and treating it as the default would make the headache, vomiting and papilledema of acute obstruction inexplicable.

High yield In normal pressure hydrocephalus gait comes first and responds best to shunting, so a patient whose gait disturbance predates the cognitive decline is the one most likely to benefit. The order of symptom onset is a selection criterion, not just a description.

Treatment: bypassing the failed pathway

A ventriculoperitoneal shunt does not repair absorption; it bypasses it. A proximal catheter sits in a lateral ventricle, passes through a one-way valve placed behind the ear, and continues under the skin of the neck and chest to the peritoneal cavity, where cerebrospinal fluid is reabsorbed by the peritoneum.

The valve is the critical component. It is pressure-set, and in modern systems often externally adjustable, so drainage tracks the pressure gradient rather than siphoning continuously. That matters because both errors are harmful: overdrainage collapses the ventricles and tears bridging veins, risking subdural hematoma, while underdrainage leaves the ventricles dilated and the symptoms in place. Setting the valve is therefore a balance rather than a maximisation.

Outcomes in well-selected patients run roughly 50 to 80 percent improvement, and the usual mechanical failures are obstruction of the catheter, disconnection of the tubing, and infection of the hardware. A patient with a shunt whose original symptoms return should be assumed to have shunt failure until proven otherwise.

Trap A shunt is not the same as the other cerebrospinal fluid procedures it is offered alongside. Coagulating the choroid plexus does cut production and is sometimes combined with endoscopic third ventriculostomy, but it is not what a shunt does. An endoscopic third ventriculostomy opens the floor of the third ventricle into the subarachnoid cisterns, which bypasses an aqueductal block from inside; it is not a laser anastomosis into a venous sinus. And blocking the aqueduct is itself a cause of obstructive hydrocephalus, so it is the problem rather than the treatment.

High yield Ventricle to peritoneum through an adjustable one-way valve. Overdrainage risks subdural hematoma from collapsed ventricles and stretched bridging veins; underdrainage leaves symptoms unchanged. Both failure modes come from the same valve setting.

Rule Treatment follows the level of failure. An intraventricular narrow point can be bypassed internally with a third ventriculostomy. A failure of absorption at the villi has to be bypassed out of the skull entirely, which is what a ventriculoperitoneal shunt does.

Memory hook Ventricle, valve, peritoneum. Three parts, three failure modes: obstruction, disconnection, infection.
