1. Orientation: the globe as three concentric tunics

Every histological section of the eye becomes readable the moment you decide which of three concentric coats you are looking at. The globe is built as a fibrous outer tunic for strength, a vascular middle tunic for blood supply and pigment, and a neural inner tunic for transduction. Put a needle through the back of the eye and you cross them in that order: dense white collagen, then a dark spongy vascular sheet, then a thin layered neural membrane, then the vitreous cavity. Identify the coat first, then name the regional specialisation within it.

High yield Fibrous tunic equals sclera plus cornea. Vascular tunic equals the uvea, which is choroid plus ciliary body plus iris. Neural tunic equals retina. A section through the posterior pole runs sclera, choroid, retina from outside in, and that single sentence orients most eye histology.

The three tunics and their regional parts

Tunic	Parts	Tissue and job
Fibrous (outer)	Sclera posterior 5/6; cornea anterior 1/6	Dense type I collagen. Resists intraocular pressure, anchors extraocular tendons, and in front refracts light
Vascular (middle, uvea)	Choroid behind; ciliary body at the equator; iris in front	Loose, melanocyte-rich, highly vascular connective tissue capped by optic-cup epithelium. Nutrition, light absorption, accommodation, aperture control, aqueous secretion
Neural (inner)	Retina, ending anteriorly at the ora serrata	Optic-cup derived. Holds photoreceptors, interneurons, ganglion cells and the pigment epithelium

The uvea is a genuine unit rather than a list to memorise. Choroid, ciliary body and iris are all the same loose, pigmented, highly vascular connective tissue, and all three are capped by derivatives of the two epithelial layers of the optic cup. That shared construction is why they inflame together as uveitis and why all three can give rise to uveal melanoma from their resident melanocytes. The sclera, by contrast, is built for tensile strength rather than blood supply, and the retina is a neuroepithelial sheet, so neither belongs to the uvea no matter how adjacent they are on a section.

Memory hook Uvea means grape: peel the white scleral skin off a globe and what you find underneath is a dark, juicy, vascular grape skin. The three parts run back to front as choroid, ciliary body, iris.

Memory hook Count the fibrous tunic in sixths: five sixths opaque sclera behind, one sixth clear cornea in front, with the limbus as the seam between them.

Sclera: why the white of the eye is white

The sclera is dense connective tissue made of flat, interlacing bundles of type I collagen with elastic fibres woven through them, scattered flattened fibroblasts between the bundles, and few vessels. At low power it is a pale, coarse, obviously fibrous band with wavy bundles running in several directions within the one field. Three features separate it from the cornea: its collagen bundles vary in diameter, they run in many directions rather than in stacked orthogonal sheets, and its ground substance differs in glycosaminoglycan composition. Each of those disturbs the regular fibril spacing that transparency requires, so incident light is scattered and the tissue looks white.

That same dense weave does mechanical work. It contains the intraocular pressure generated by aqueous secretion, maintains the shape of the globe so the optics stay aligned, and provides the insertion for the extraocular tendons, which blend into its superficial layers. Posteriorly it is perforated by the axons of the optic nerve and by the central retinal vessels; anteriorly it becomes continuous with the cornea.

Trap Loose, highly vascular, pigmented connective tissue is the choroid, not the sclera. The two lie directly against one another, so a section at the posterior pole shows both, and the quickest discriminator is pigment plus capillaries versus pale avascular bundles.

Trap The tough outer coat is not cartilage and not muscle. Elastic cartilage in this region belongs to the pinna, the external auditory meatus and the epiglottis, and the only smooth muscle inside the globe is the ciliary muscle plus the sphincter and dilator pupillae.

Two landmarks that tell you where you are: limbus and ora serrata

Two transition zones carry most of the regional anatomy, and both are favourite points to be tested on because at each one several tissues change character at once.

The limbus, the corneoscleral junction Here the regular, orthogonally stacked corneal lamellae give way to the irregular, variably sized scleral bundles, so transparency ends abruptly. The conjunctival stratified columnar epithelium becomes corneal stratified squamous epithelium, and the stem cells that resurface the cornea live in this epithelium. Deep to the surface sits the iridocorneal angle with the trabecular meshwork and the canal of Schlemm, the drainage apparatus for aqueous humour. Limbal capillaries also nourish the peripheral cornea.
The ora serrata, the anterior limit of the neural retina This scalloped line is where the photoreceptor-bearing retina stops and meets the ciliary body. Behind it you can count every retinal layer. In front of it the retina thins abruptly to just the two epithelial layers of the optic cup, which continue forward over the ciliary body as the pigmented and non-pigmented ciliary epithelium and over the back of the iris as the iris pigment epithelium.

Rule Both layers of the optic cup run the whole way forward to the pupillary margin, but only behind the ora serrata does the inner layer differentiate into a multilayered neural retina. In front of the ora serrata the inner layer stays a single simple epithelium.

Rule The limbus is a corneal and scleral landmark; the ora serrata is a retinal and ciliary landmark; the optic disc is the posterior exit point for ganglion cell axons and the entry point for the central retinal vessels. Three different places, three different jobs.

Trap Do not confuse the ora serrata with the rim of the fovea, where the displaced inner retinal layers pile up again, and do not confuse it with the limbus. Only one of the three, the ora serrata, marks the end of photoreceptors.

Named membranes of the eye and ear, kept straight

A disproportionate share of errors in this region comes from a handful of eponymous membranes with similar names sitting in completely different places. Fix them once, as a set, with the tissue that made each one.

Membrane	Location and maker	Key point
Bowman's membrane	Cornea, just deep to the epithelial basal lamina	Acellular condensation of randomly arranged collagen, not a true basement membrane. Does not regenerate, so wounds through it scar. Limits the spread of infection and tumour
Descemet's membrane	Cornea, secreted by the corneal endothelium that rests on it	The thickest basement membrane in the body, rich in type IV collagen and laminin, PAS positive. Nodular excrescences of it are the guttae of Fuchs dystrophy
Bruch's membrane	Between the choriocapillaris and the retinal pigment epithelium	Part of the outer blood-retina barrier and the plane where drusen collect in macular degeneration; new vessels that breach it cause wet macular degeneration
Basilar membrane	Cochlea, separating scala media from scala tympani	Carries the organ of Corti. Stiff and narrow at the base for high frequencies, floppy and wide at the apex for low ones

Trap Bowman's and Descemet's are both corneal and both acellular lines, but only Descemet's is a real basal lamina with a cell layer sitting on it. If the stem asks for a basement membrane, Bowman's is never the answer; if it asks what limits infection spread beneath the epithelium, Descemet's is never the answer.

Trap Bruch's membrane and the basilar membrane are not even in the same organ. Bruch's is retinal and sits under the pigment epithelium; the basilar membrane is cochlear and sits under the hair cells. Both get offered as plausible-looking distractors for the thickest basement membrane, which is Descemet's.

A tissue census for the eye

Because so many prompts in this region reduce to naming a tissue, it pays to hold the census in one place. Dense collagen in and around the globe means sclera, corneal stroma and the tarsal plate of the eyelid. Loose vascular pigmented connective tissue means the uvea. Stratified epithelia are surface linings: cornea and conjunctiva at the front of the eye, plus the keratinised epidermis of the outer eyelid and the external auditory meatus. Simple squamous sheets mean the corneal endothelium and the endothelium of blood vessels, both built for pumping and diffusion across a very thin layer. Smooth muscle means the ciliary muscle and the two pupillary muscles. Skeletal muscle in the orbit means the extraocular muscles and levator palpebrae superioris, and in the middle ear the stapedius and tensor tympani.

High yield Goblet cells in this region occur in two places only: the conjunctiva, where their mucin forms the innermost layer of the tear film, and the nasopharyngeal end of the eustachian tube, where the lining is continuous with respiratory mucosa. They are never part of the ciliary epithelium, which secretes a watery aqueous humour, and never part of the skin-lined external auditory meatus.

Transitional epithelium is worth one explicit exclusion: urothelium stretches as the bladder fills and occurs nowhere in the eye or the ear. When transitional appears as an option for an ocular surface, it is pure noise.

Rule Name the tissue by the job it has to do. Transparency demands avascular, non-keratinised and regularly spaced; strength demands dense irregular collagen; absorption of stray light demands melanin; fluid secretion demands a tall mitochondria-rich epithelium with tight junctions; mechanotransduction demands hair cells on a membrane bathed in a potassium-rich fluid.

2. The cornea, layer by layer, and the physics of clarity

The cornea reads as a five-layer sandwich, and the order is easiest to hold if you follow the tissue that built each layer rather than memorising five names. Tears meet an epithelium; that epithelium lays down a basal lamina which blends into the front of the stroma as Bowman's membrane; the stroma occupies almost the whole thickness; and at the back a single sheet of endothelium secretes its own basal lamina, Descemet's membrane, which therefore always lies immediately outside the endothelium, facing the anterior chamber.

High yield Five layers, outside in: epithelium, Bowman's membrane, stroma or substantia propria, Descemet's membrane, endothelium. Of the two acellular lines, only Descemet's is a true basement membrane, and it is the thickest basement membrane in the body.

Memory hook Work outside in and let the cells tell you where their membranes go. Epithelium sits on Bowman's; endothelium sits on Descemet's. The stroma is the filling between the two membranes.

Layer one: the corneal epithelium

The corneal surface is non-keratinised stratified squamous epithelium about five cell layers deep, and each stratum looks different at high power. The surface cells are flattened, carry microvilli coated in glycocalyx, and are sealed to one another by tight junctions. Beneath them sit two or three layers of polygonal wing cells. The deepest layer is basal and cuboidal, resting on a basal lamina. The whole sheet turns over within days, resurfaced from stem cells that live in the limbal epithelium at the corneal rim.

Every one of those features serves either transparency or protection. Keratin would scatter light and make the cornea opaque, so the surface is deliberately non-keratinised and instead stays wet, glycocalyx coated and tightly sealed, while stratification alone provides the abrasion resistance that a blinking surface needs. The epithelium is also the most densely innervated epithelium in the body, supplied by free nerve endings from the ophthalmic division of the trigeminal nerve, which is why even a superficial abrasion is so painful and why the blink and tearing reflexes are so brisk.

Trap Simple squamous is a single flattened layer built for diffusion or a low-friction lining, which is what the corneal endothelium and vascular endothelium use. A surface exposed to blinking and abrasion cannot be a single flattened layer, so the front of the cornea is never simple squamous.

Trap Keratinised stratified squamous epithelium is the dry, dead-cell-topped lining of skin, including the outer surface of the eyelid and the external auditory meatus. On the cornea it is pathological, as in vitamin A deficiency, because the keratin scatters light and clouds the surface.

Trap Pseudostratified ciliated columnar epithelium with goblet cells is respiratory lining. In this region it belongs to the nasal cavity and the nasopharyngeal end of the eustachian tube, where mucus must be swept along, not to any ocular surface.

Layer two: Bowman's membrane

Bowman's membrane is a thin, acellular zone of randomly arranged collagen immediately deep to the epithelial basal lamina, best thought of as modified anterior stroma rather than as a membrane in its own right. It is not a true basement membrane: no cell layer rests on it and it is not the product of a single epithelium. It does two things. It acts as a barrier that limits the spread of infection and of tumour into the stroma, and it fails to regenerate once breached, which is the histological reason that a wound deep enough to cross Bowman's heals with a scar while a purely epithelial abrasion heals clear.

Layer three: the stroma, and why the cornea is transparent

The stroma, or substantia propria, is roughly 90 percent of corneal thickness and dominates the field as a pale, uniform, fibrous band with flat keratocyte nuclei squeezed between the sheets. It consists of some 200 to 250 lamellae of type I collagen. Within any single lamella the fibrils lie parallel to one another; each lamella then runs at right angles to the next, and the whole stack is set in a keratan sulfate-rich ground substance. The flattened keratocytes are the resident fibroblast-like cells that maintain this matrix, and there are no blood vessels and no lymphatics anywhere in it.

The orthogonal lattice is the optical device. By holding collagen fibril spacing and fibril diameter below the wavelength of visible light, the lattice ensures that the light scattered by neighbouring fibrils interferes destructively and cancels out, so almost all of the incident light passes straight through. Anything that disturbs that spacing destroys the effect. Stromal oedema is the clearest example: water drawn in between the lamellae pushes the fibrils apart, the cancellation fails, and the tissue goes hazy. Vessels growing into the stroma do the same thing, both by their own opacity and by the oedema and scarring that accompany them.

Layer	Histology	Functional point
Epithelium	Non-keratinised stratified squamous, about five layers: flat surface cells with microvilli, wing cells, basal cuboidal cells on a basal lamina	Barrier and abrasion resistance; most densely innervated epithelium in the body via the ophthalmic division of the trigeminal nerve; renewed within days from limbal stem cells
Bowman's membrane	Thin acellular zone of randomly arranged collagen; not a true basement membrane	Limits spread of infection and tumour; does not regenerate, so deeper wounds scar
Stroma	About 90 percent of thickness; 200 to 250 orthogonal type I collagen lamellae in keratan sulfate-rich matrix with flat keratocytes; avascular	Transparency by sub-wavelength fibril spacing; tensile strength; swelling scatters light
Descemet's membrane	Homogeneous, strongly eosinophilic, PAS positive line of type IV collagen and laminin, about 3 micrometres at birth and thickening with age	Basal lamina of the endothelium; thickest basement membrane in the body; focal excrescences are guttae
Endothelium	Single sheet of flattened polygonal cells with tight and gap junctions, abundant mitochondria and sodium-potassium ATPase	Pumps water out of the stroma to keep it clear; does not divide in adults

Trap The epithelium is only about five cell layers, roughly a tenth of corneal thickness, and Descemet's, though the thickest basement membrane in the body, is still only a few micrometres thick. Neither can be the layer that accounts for 90 percent of the cornea. Only the stroma is in the running.

Layers four and five: Descemet's membrane and the endothelium

Descemet's membrane is the basal lamina of the corneal endothelium and the thickest basement membrane in the body, roughly 3 micrometres at birth and thickening steadily through life. On haematoxylin and eosin it is a sharp, homogeneous, strongly eosinophilic line, and it is PAS positive because it is packed with type IV collagen and laminin. The contrast to carry is that Bowman's is acellular modified stroma beneath the epithelium while Descemet's is a true basal lamina made by the cells sitting on it.

The endothelium itself is a single sheet of flattened polygonal cells facing the anterior chamber, joined by tight junctions and gap junctions and loaded with mitochondria and sodium-potassium ATPase. It runs a pump-and-leak system: aqueous humour continually seeps into the stroma down its osmotic and hydrostatic gradients, and the endothelium actively pumps that fluid back out. The stroma is therefore held in a relatively dehydrated state in which the collagen lattice keeps its regular spacing and stays clear. These cells do not divide in adults, so when cells are lost the gaps are covered only by neighbours spreading thinner and wider, and endothelial cell density falls irreversibly. Once density drops below what the pump needs, the stroma imbibes water and the cornea clouds, which is what happens in Fuchs endothelial dystrophy and after intraocular surgery.

Rule Corneal clarity is a four-part contract: avascular stroma, non-keratinised surface, regularly spaced orthogonal lamellae, and relative stromal dehydration maintained by the endothelial pump. Break any one of the four and the cornea clouds.

Rule Descemet's membrane always lies just outside the endothelium, between it and the stroma. Use that to anchor the back two layers whenever an ordering prompt tries to invert them.

Trap The endothelium does not secrete aqueous humour; it only pumps fluid back out of the stroma. Aqueous humour is secreted by the non-pigmented layer of the ciliary epithelium on the ciliary processes. Nor does the cornea make tears, which come from the lacrimal gland, conjunctival goblet cells and the meibomian glands, and nor does it anchor the zonule, which runs from the non-pigmented ciliary epithelium to the lens capsule.

Nutrition, innervation and immune privilege

Because it has no vessels, the cornea is nourished entirely by diffusion from three directions: the tear film in front, the aqueous humour behind, and the limbal capillaries at its rim. That avascularity is also the basis of corneal immune privilege, which is why corneal grafting is so successful and why donor and recipient are usually not matched at all. With no lymphatics crossing the graft bed, donor antigens never reach regional lymph nodes to prime a response, and with no capillaries, host lymphocytes have no easy route to the graft. The corollary follows directly: anything that drives new vessels into the cornea, such as chronic hypoxia from contact lens wear, both clouds the tissue and converts an immune-privileged site into a vascularised one with a much higher rejection risk.

High yield No vessels, no lymphatics, no node drainage, so the cornea is immune privileged and grafts take without HLA matching. The nerves, by contrast, are abundant: the cornea is the most densely innervated tissue in the body.

Trap Avascular does not mean denervated. A prompt that explains graft survival by an absence of nerves has it backwards, since the cornea is the most densely innervated tissue in the body. Likewise the cornea is mostly collagen, about 90 percent of its thickness being type I lamellae with type IV collagen in Descemet's, so lacking collagen is never the explanation for anything corneal.

Cornea against sclera at the bench

Feature	Cornea	Sclera
Share of fibrous tunic	Anterior one sixth	Posterior five sixths
Collagen arrangement	Uniform-diameter type I fibrils in parallel lamellae stacked at right angles	Flat interlacing bundles of varied diameter running in many directions, with elastic fibres
Cells	Flat keratocytes in the stroma, epithelium in front, endothelium behind	Scattered flattened fibroblasts
Vascularity and appearance	Avascular and transparent; fed by tears, aqueous and limbal capillaries	Few vessels and opaque white because light is scattered

At the limbus the two merge, and a section cut across it shows the switch in a single field: regular stacked lamellae on the corneal side, irregular interlacing bundles on the scleral side, with the trabecular meshwork and canal of Schlemm in the depths of the angle and the limbal epithelial stem cells at the surface.

High yield Both cornea and sclera are type I collagen, so composition does not explain the difference in appearance. Architecture does: regular sub-wavelength spacing transmits light, irregular bundles of mixed diameter scatter it.

3. The uvea: iris, ciliary body, choroid and the aqueous circuit

The vascular tunic does four jobs that a single tissue type could not do alone, so it specialises regionally. In front, the iris is an adjustable aperture with its own two muscles. At the equator, the ciliary body carries the muscle of accommodation and the epithelium that secretes aqueous humour. Behind, the choroid is a nutritional and light-absorbing bed for the outer retina. The connective tissue core is the same throughout: loose, highly vascular, and full of melanocytes.

High yield The non-pigmented layer of the ciliary epithelium secretes aqueous humour and forms the blood-aqueous barrier. The choriocapillaris feeds the outer retina. The iris pigment epithelium sets the light-tight backing of the iris. All three are optic-cup epithelium sitting on uveal connective tissue.

The iris: stroma, pigment epithelium and the colour of the eye

A radial section of the iris shows a loose, vascular, melanocyte-containing stroma in front and, plastered across its posterior surface, the two layers of the iris pigment epithelium, both so densely melanised that on haematoxylin and eosin they look almost black. Those two layers are the forward continuation of the optic-cup epithelium that covers the ciliary body, and they form a light-tight backing so that light can enter the eye only through the pupil.

Eye colour depends on the quantity of melanin held in the iris, not on any additional pigment. The posterior pigment epithelium is always heavily melanised; what varies between individuals is melanin content in that epithelium and in the melanocytes of the loose anterior stroma. Abundant melanin absorbs the light entering the stroma and the iris looks brown. Sparse melanin lets short wavelengths scatter back out of the stroma, which the eye perceives as blue for the same physical reason a cloudless sky is blue. In albinism no melanin is made at all, so the choroidal and iridal vessels show through and the iris looks pink.

Trap Blue irides contain no blue pigment, and green and hazel irides contain no separate green pigment. There is one pigment, melanin, and only its amount varies. A prompt offering a different pigment type is testing exactly this.

Trap Corneal thickness, about half a millimetre centrally, matters for refraction and for recognising oedema, but the cornea is transparent and contributes no colour. Choroidal blood produces the red of the fundus and the pink iris of albinism but does not generate brown or blue irides. Lens crystallins keep the lens clear and their oxidation causes cataract; they have nothing to do with iris colour.

The two pupillary muscles and how to tell them apart on a section

Both pupillary muscles are smooth muscle, and unusually for smooth muscle both arise from neuroectoderm rather than mesoderm, because they develop from the outer layer of the optic cup. On a radial section through the iris you distinguish them by the plane in which their fibres have been cut.

Feature	Sphincter pupillae	Dilator pupillae
Arrangement	Compact band of circular bundles at the pupillary margin	Thin radial sheet behind the stroma running out to the iris root
Appearance on a radial section	Cut across the fibres, so it appears as small rings or whorls	Cut lengthwise, so it appears as a long thin longitudinal band
Innervation	Parasympathetic from CN III relaying in the ciliary ganglion	Sympathetic
Action	Miosis, constricting the pupil	Mydriasis, dilating the pupil

Memory hook Circular equals constrictor equals parasympathetic; radial equals dilator equals sympathetic. Cut a ring across and you get rings; cut a radial spoke along its length and you get a band.

High yield A sympathetic lesion in Horner syndrome gives miosis, because the radial dilator is paralysed and the parasympathetic sphincter wins, together with ptosis from loss of the sympathetically driven superior tarsal muscle. Antimuscarinic drugs block the parasympathetic sphincter and so dilate the pupil.

Trap Reversing the pair is the classic error: if the circular muscle were sympathetic and the radial one parasympathetic, atropine would constrict the pupil, which it plainly does not. The two differ in both fibre orientation and innervation, and that double difference is what makes the pupillary reflex clinically testable. Neither is skeletal muscle; skeletal muscle in the orbit means the extraocular muscles and levator palpebrae superioris.

The ciliary body: muscle for accommodation

The ciliary body is a triangular wedge of uveal connective tissue at the equator of the globe, and its bulk is the ciliary muscle, arranged in longitudinal, radial and circular bundles. It is smooth muscle: on section the cells are pale and spindle-shaped with a single central nucleus and no cross-striations, which separates them immediately from the multinucleate striated fibres of the extraocular muscles lying just outside the sclera. Its motor supply is parasympathetic. Preganglionic fibres leave the brainstem in CN III, synapse in the ciliary ganglion, and the postganglionic fibres arrive as the short ciliary nerves.

The mechanics are counterintuitive and worth stating as a sequence, because the common error is to assume that a contracting muscle must pull the lens flatter.

At rest The relaxed ciliary muscle leaves the ciliary ring at its widest, so the zonule fibres are taut and pull outward on the lens equator, holding the elastic lens flattened for distance vision.
Parasympathetic activation The ciliary muscle contracts, drawing the ciliary body forward and inward and reducing the diameter of the ring.
Zonular slackening With the ring narrowed, tension on the zonule falls.
Lens rounding Released from outward tension, the intrinsically elastic lens rounds up, increasing its refractive power for near vision. This is accommodation.

Rule Ciliary muscle contraction slackens the zonule and the lens becomes more convex. Relaxation tightens the zonule and the lens flattens. Accommodation is therefore an active parasympathetic event, and blocking muscarinic receptors produces cycloplegia, the loss of accommodation.

Rule Intraocular smooth muscle always needs a postganglionic relay, which for the ciliary muscle and the sphincter pupillae is the ciliary ganglion. Direct somatic supply from CN III without a relay belongs to the skeletal extraocular muscles such as medial rectus.

Presbyopia follows from the histology of the lens rather than from the muscle: as lens fibres accumulate and the lens stiffens with age, the same zonular slackening no longer allows the lens to round up, so the near point recedes even though the ciliary muscle is working normally.

Trap The sympathetically driven smooth muscles of the eye are the dilator pupillae and the superior tarsal muscle. A sympathetic lesion therefore gives a small pupil and a drooping lid, never a loss of accommodation. CN VII supplies no intraocular muscle at all; its targets here are orbicularis oculi for lid closure and stapedius in the middle ear.

The ciliary processes and the secretion of aqueous humour

The inner surface of the ciliary body is thrown into some seventy radial ridges, the ciliary processes, each a vascular core of uveal connective tissue covered by a double epithelium. That double epithelium is the forward continuation of the two optic-cup layers, and the two layers lie apex to apex: an outer pigmented layer against the stroma and an inner non-pigmented layer facing the posterior chamber.

The non-pigmented cells are the secretory ones and they look it. They have extensive basolateral membrane infoldings, numerous mitochondria, abundant carbonic anhydrase, and apical tight junctions that constitute the blood-aqueous barrier. They actively secrete aqueous humour into the posterior chamber rather than simply filtering it, which is why drugs that inhibit carbonic anhydrase lower intraocular pressure by acting at exactly this epithelium. The zonule fibres of the lens anchor to the basal lamina of these same non-pigmented cells.

Trap The ciliary epithelium is a double layer of cuboidal to columnar cells, not a simple squamous endothelium, not a stratified squamous surface, and not a goblet-cell epithelium. Simple squamous endothelium lines vessels and the back of the cornea; stratified squamous covers the cornea, the conjunctiva near the lid margin and the external auditory meatus; goblet cells secrete mucin, whereas aqueous humour is watery.

The aqueous circuit, start to finish

Secretion The non-pigmented ciliary epithelium on the ciliary processes actively secretes aqueous humour.
Posterior chamber The fluid enters the narrow slot between the posterior surface of the iris and the lens, where it bathes and nourishes the avascular lens.
Through the pupil It passes forward through the pupillary aperture.
Anterior chamber Between iris and cornea it nourishes the corneal endothelium and supplies the posterior cornea by diffusion, then flows peripherally to the iridocorneal angle.
Trabecular meshwork and canal of Schlemm At the angle it filters through the trabecular beams into the canal of Schlemm, which encircles the limbus.
Venous blood From Schlemm's canal it drains into the episcleral veins and joins the systemic circulation.

High yield Obstruction of outflow at the iridocorneal angle, at the trabecular meshwork or at the canal of Schlemm raises intraocular pressure and causes glaucoma, which damages the optic nerve head. The lesion of glaucoma is in the drainage apparatus, not in the secreting epithelium and not in the lens.

Rule Aqueous humour is an intraocular fluid that nourishes the lens and the corneal endothelium and then leaves at the angle. It never reaches the tear film, and the tear film never enters the eye.

The choroid and the choriocapillaris

The choroid is loose, highly vascular connective tissue: a dark, spongy sheet between the sclera and the retinal pigment epithelium, packed with vessels of every calibre and with plump melanocytes that give it a brown-black colour in the fresh state and a heavily pigmented appearance on a section. Its melanin absorbs stray light that has passed through the retina, preventing internal reflection that would otherwise blur the image.

Its innermost vascular layer is the choriocapillaris, a dense sheet of wide fenestrated capillaries separated from the retinal pigment epithelium only by Bruch's membrane. That arrangement is forced by the retina itself: the photoreceptors and the whole outer retina have no vessels of their own, so they must be fed by diffusion across Bruch's membrane from this fenestrated bed. The combination of fenestrated capillaries for delivery, Bruch's membrane as the diffusion barrier, and the tight junctions of the pigment epithelium as the actual seal constitutes the outer blood-retina barrier.

High yield Uveal melanocytes are the source of uveal melanoma, which can arise in choroid, ciliary body or iris. New vessels growing from the choriocapillaris through Bruch's membrane cause wet age-related macular degeneration and sudden loss of central vision.

Trap The choroid is connective tissue, not an epithelium, so stratified epithelium is never the right description for it. Dense collagenous tissue in this region means sclera, corneal stroma or tarsal plate, and smooth muscle sheets in the uvea are confined to the ciliary muscle and the two pupillary muscles. Separation of the choroid from the sclera is a choroidal detachment filling the potential suprachoroidal space, an entirely different plane from retinal detachment.

4. The retina: layers, photoreceptors and the pigment epithelium

The retina is the one place in the body where a laminated central nervous system structure can be examined through an intact window, and its layers are named from the choroid inward. That order is the developmental order, following the optic cup from its outer layer inward, and it is the exact reverse of the path light takes. Light entering the eye crosses the whole thickness of the retina backwards, passing the ganglion cells first and reaching the photoreceptors last, before being absorbed by the pigment epithelium at the very back.

High yield Layers outside in: pigment epithelium, photoreceptor outer and inner segments, outer nuclear layer, outer plexiform layer, inner nuclear layer, inner plexiform layer, ganglion cell layer, with the ganglion cell axons sweeping inward to the optic disc. Light crosses them in reverse, hitting the ganglion cells first.

Memory hook Nuclei in the nuclear layers, synapses in the plexiform layers, and they strictly alternate. Outer means nearer the choroid, inner means nearer the vitreous, so every pair of nuclear and plexiform layers sits beside the layer it synapses into.

The layers in order, and what each band contains

Layer, outside in	Contents	What it does
Retinal pigment epithelium	Single layer of pigmented cuboidal cells on Bruch's membrane	Absorbs stray light, phagocytoses shed discs, recycles retinal, forms the outer blood-retina barrier
Photoreceptor segments	Rod and cone outer and inner segments, sheathed by pigment epithelial microvilli	Phototransduction in the stacked discs of the outer segment
Outer nuclear layer	Nuclei of rods and cones	Cell bodies of the photoreceptors whose segments lie just outside
Outer plexiform layer	Synapses of photoreceptors onto bipolar and horizontal cells	First synaptic relay and the site of lateral inhibition by horizontal cells
Inner nuclear layer	Nuclei of bipolar, horizontal, amacrine and Mueller cells	Interneurons plus the radial glia of the retina
Inner plexiform layer	Synapses of bipolar cells onto ganglion and amacrine cells	Second synaptic relay and the site of lateral interaction by amacrine cells
Ganglion cell layer	Large ganglion cell bodies, innermost band of nuclei	Output neurons; their axons converge at the optic disc and become the optic nerve

The inner nuclear layer repays close reading because it holds four quite different cell populations. Bipolar cells carry the signal radially inward from photoreceptor to ganglion cell. Horizontal cells spread laterally within the outer plexiform layer, sharpening contrast at the first synapse. Amacrine cells spread laterally within the inner plexiform layer, shaping the signal at the second synapse. Mueller cells are the radial glia of the retina, and their nuclei sit here while their processes span the full retinal thickness from the outer limiting zone to the vitreal surface, providing structural and metabolic support.

Trap Rod and cone nuclei are not in the inner nuclear layer; they lie in the outer nuclear layer, immediately inside their own segments. Ganglion cells have their own innermost band. Pigment epithelial cells make up the single outermost cuboidal layer on Bruch's membrane. Only the interneurons and Mueller glia occupy the inner nuclear layer.

Rule Each nuclear layer holds the cell bodies whose synapses fill the plexiform layer beside it. Apply that rule and you can reconstruct the lamination from first principles rather than from a list.

The retinal pigment epithelium

The retinal pigment epithelium is a single layer of cuboidal cells resting on Bruch's membrane, and its polarity is written all over it. Apically it carries melanin granules and long microvilli that reach up and sheathe the tips of the photoreceptor outer segments. Basally it has extensive membrane infoldings against Bruch's membrane, the hallmark of a transporting epithelium. Laterally, neighbouring cells are joined by tight junctions. Four functions follow directly from that structure.

Light absorption Apical melanin absorbs light that has passed the photoreceptors, preventing internal reflection and the blur it would produce.
Phagocytosis The microvilli engulf shed outer-segment discs, clearing roughly a tenth of each photoreceptor's discs every day, which is how the photoreceptor renews its light-sensitive membrane for a lifetime.
The visual cycle These cells re-isomerise all-trans retinal back to 11-cis retinal and return it to the photoreceptors, so the pigment can be regenerated after bleaching.
Barrier The tight junctions between pigment epithelial cells, together with Bruch's membrane beneath them, form the outer blood-retina barrier that keeps the fenestrated choriocapillaris from leaking into the subretinal space.

Trap The pigment epithelium senses no light. Colour is detected by the three cone types and their opsins; the pigment epithelium only supports them by recycling retinal. It does not secrete aqueous humour, which comes from the non-pigmented ciliary epithelium, and it does not form the optic nerve, which is made of ganglion cell axons.

Rods against cones

At high power the two photoreceptor types are visibly different. Rods are slender, with a long cylindrical outer segment made of stacked discs that have pinched off from the plasma membrane and float free inside it. Cones are stubbier and conical, with a shorter outer segment whose discs remain continuous with the plasma membrane as infoldings. The differences in number, pigment and wiring then explain their complementary roles.

Feature	Rods	Cones
Number	About 120 million	About 5 million
Pigment	A single pigment, rhodopsin	Three different opsins, one per cone type
Outer segment	Long and cylindrical with free stacked discs	Shorter and conical with discs continuous with the plasma membrane
Wiring and performance	Many rods converge onto one bipolar cell, so sensitivity is superb but resolution is low and there is no colour discrimination	Nearly one-to-one wiring, so acuity and colour discrimination are high but more light is required
Distribution	Absent from the fovea, peak density about 20 degrees out in the peripheral retina	Highest density in the fovea, which contains cones only

High yield Rods outnumber cones by roughly twenty to one, about 120 million against about 5 million, yet cones dominate central vision because the fovea is cone-only and cones wire nearly one-to-one.

Trap The commonest reversal is to give rods colour vision and cones night vision. Cones carry three opsins and work in bright light for colour; rods carry the single pigment rhodopsin and serve night and peripheral vision. The two also have almost opposite distributions, so they are never both concentrated in the fovea.

The fovea and the macula lutea

The fovea centralis is a shallow pit at the centre of the macula lutea, and three histological features acting together make it the sharpest point in the visual field.

Cones only, at maximum density There are no rods, and the cones here are unusually slender so that more of them fit per unit area.
Inner layers displaced The bipolar and ganglion cell bodies and the retinal vessels that normally lie in the light path are swept aside to the rim of the pit, so light reaches the cone outer segments almost unobstructed.
Avascular and nearly one-to-one No retinal vessels cross it; it is fed by diffusion from the choriocapillaris. Each foveal cone drives close to a single bipolar and ganglion cell, so fine spatial detail is preserved rather than summed away.

Trap The fovea is the thinnest part of the retina, precisely because its inner layers have been displaced sideways. The retina is thickest near the optic disc. A prompt that explains foveal acuity by extra retinal thickness is inverted.

Trap A rod-only region would be exquisitely light-sensitive but low-resolution and colour-blind, which describes the peripheral retina, not the fovea. And the optic disc is a separate structure about 3 mm nasal to the fovea; it holds no photoreceptors at all.

The optic disc and the blind spot

The optic disc is the point at which ganglion cell axons converge, turn posteriorly through the sclera to form the optic nerve, and acquire myelin, and at which the central retinal artery and vein enter and leave the eye. Histologically it consists of nothing but axons, glia and vessels. There are no photoreceptors, no bipolar cells and no pigment epithelium, so no light is transduced there and the corresponding part of the visual field is the physiological blind spot. On fundus examination it appears as a pale, cupped disc about 3 mm nasal to the fovea with the retinal vessels radiating out of it.

High yield The blind spot exists because the disc has no photoreceptors of any kind. Raised intracranial pressure obstructs axoplasmic flow in those converging axons and swells the disc, which is papilledema.

Trap The disc is not blind for lack of blood supply. It is exactly where the central retinal artery and vein enter and leave, so it is richly supplied; the avascular zone of the retina is the fovea. Nor is it covered by the lens, which sits far anteriorly just behind the iris and covers no part of the retinal surface. And an area with only rods, such as peripheral retina, still sees in dim light.

Retinal detachment and the plane of weakness

Retinal detachment separates the neural retina from the retinal pigment epithelium, and the reason that particular plane gives way is developmental. The optic cup forms as an invagination with an inner layer that becomes the neural retina and an outer layer that becomes the pigment epithelium, with the intraretinal space between them. That space is obliterated as the cup closes, but the two sheets never truly fuse: they are held together only by the interdigitation of pigment epithelial microvilli with the photoreceptor tips and by the pumping action of the epithelium, which keeps the subretinal space dry. The old intraretinal space therefore remains the weakest plane in the retina for life.

The same plane separates readily during tissue preparation, so lifting of the photoreceptor segments away from the pigmented layer, which stays behind on Bruch's membrane, is a very common artefact. Clinically the consequence is severe: once detached, the photoreceptors are cut off from the choriocapillaris, their only source of nutrition and the only route for recycling retinal, and they die.

Trap Three other separations are offered as look-alikes and none of them is retinal detachment. Choroid from sclera is a choroidal detachment or effusion in the suprachoroidal space. Cornea from sclera cannot occur because the two are continuous collagenous tissue joined at the limbus. Separation of the vitreous from the retinal surface is posterior vitreous detachment, which causes floaters and can tear the retina and thereby lead to a true retinal detachment.

Rule Anything that names the pigment epithelium on one side and the photoreceptors on the other is describing the embryological intraretinal space, and that is retinal detachment. Anything that names sclera on one side is describing a different, more external plane.

Where the retina stops

Anteriorly the laminated retina ends at the ora serrata, the scalloped line at which it meets the ciliary body. Behind that line all the layers described above can be counted. In front of it the retina thins abruptly to just the two optic-cup epithelial layers, which run forward as the pigmented and non-pigmented ciliary epithelium and then as the two layers of the iris pigment epithelium. Vision, therefore, stops at the ora serrata even though the epithelium continues.

High yield Ora serrata equals the anterior limit of photoreceptors. Beyond it the optic cup persists only as a double epithelium, which is why the ciliary processes and the back of the iris carry two epithelial layers lying apex to apex.

5. Lens, zonule, lids, conjunctiva and the lacrimal apparatus

The refractive and protective accessories of the eye are all epithelial organs or epithelial derivatives, and they share one theme: each depends on a specialised secretion or a specialised loss of structure to do its job. The lens achieves transparency by destroying its own organelles. The zonule achieves extensibility by using a fibre of the elastic system with no elastin core. The tear film achieves stability by layering three secretions from three different glands with three different modes of secretion.

High yield The lens is capsule plus anterior simple cuboidal epithelium plus anucleate crystallin-filled fibres, with no vessels and no nerves. The zonule is oxytalan, that is fibrillin-rich microfibrils. The tear film is lipid over aqueous over mucin, from meibomian glands, the lacrimal gland and conjunctival goblet cells respectively.

The lens: three components, one optical requirement

The lens is an epithelial organ with exactly three components, and a section through its anterior pole shows all three in sequence.

The capsule A thick, homogeneous, strongly PAS-positive basal lamina of type IV collagen and laminin that completely invests the lens. It is made by the lens cells themselves and is the structure a surgeon leaves behind when implanting an artificial lens.
The lens epithelium A simple cuboidal epithelium covering the anterior surface only, immediately beneath the capsule. Its cells divide at the equator, which is the only germinal zone the lens has.
The lens fibres The daughters of those equatorial divisions elongate enormously, lose their nuclei and all their organelles, and fill with crystallins. They are laid down in concentric lamellae, so the oldest fibres sit in the core and the newest at the periphery. On section they are long, pale, anucleate, eosinophilic ribbons.

Transparency depends on that programmed self-destruction. An organelle-free, nucleus-free cytoplasm uniformly filled with crystallins has no internal membranes to scatter light. The price is that lens fibres can never be replaced and their crystallins must last a lifetime. The lens is also entirely avascular and has no nerves, so it is nourished only by the aqueous humour bathing it in the posterior chamber.

Trap The lens is not stratified squamous epithelium, not dense collagen and not muscle. Stratified squamous covers surfaces exposed to wear such as the cornea, the conjunctiva near the lid margin and the external auditory meatus. Dense collagen builds the sclera, the corneal stroma and the tarsal plate, while the lens has only a thin type IV collagen capsule. The muscle that changes lens shape is the ciliary muscle in the ciliary body; the lens itself is passive and purely epithelial.

Cataract: a disease of proteins that are never replaced

Cataract is opacification of the lens itself. Because lens fibres are made once and never renewed, cumulative oxidation, glycation and ultraviolet damage progressively cross-link and aggregate the crystallins. Aggregated protein disrupts the orderly, uniform packing on which transparency depends and begins to scatter light. Histologically the regular lamellar pattern is lost and the fibres appear swollen, fragmented and broken up into rounded eosinophilic bodies, the Morgagnian globules.

The risk factors read directly off the chemistry: ageing, which simply means more time for cross-linking; diabetes, which supplies glycation; corticosteroids; smoking; and ultraviolet exposure.

Trap Keep the three classic opacities in their own compartments. Cataract is the lens and is a protein problem. Glaucoma is obstructed aqueous outflow at the trabecular meshwork and canal of Schlemm, a pressure problem that damages the optic nerve head. Corneal oedema is a waterlogged, hazy corneal stroma caused by failed endothelial pumping, as in Fuchs endothelial dystrophy. Choroidal neovascularisation through Bruch's membrane is wet macular degeneration and costs central vision.

The zonule: oxytalan microfibrils under tension

The zonule fibres, collectively the suspensory ligament of the lens, are bundles of oxytalan microfibrils. Oxytalan means fibrillin-rich microfibrils of the elastic fibre system that lack an elastin core, so the fibres are extremely fine, stain poorly with haematoxylin and eosin and are demonstrated with elastic stains such as orcein. They run from the basal lamina of the non-pigmented ciliary epithelium to the lens capsule near the equator, holding the lens centred in the visual axis and keeping it under the outward tension that the ciliary muscle intermittently releases for accommodation.

High yield Fibrillin is the key protein of the zonule, so disorders of fibrillin subluxate the lens. In Marfan syndrome the lens dislocates upward; in homocystinuria it dislocates downward.

Trap Type I collagen is the tensile fibre of sclera, corneal stroma, tendon and bone, and it is far too coarse and inextensible to serve as a suspensory ligament that must stretch and recoil with every accommodative effort. Elastic cartilage belongs to the pinna, the external auditory meatus and the epiglottis, and keratins are epithelial intermediate filaments, piled up as the tough surface of epidermis including the skin of the eyelid.

The conjunctiva

Conjunctival epithelium is stratified columnar, only two to five cells thick, with abundant goblet cells scattered through it. It rests on a loose, vascular, lymphocyte-rich lamina propria, the substantia propria, whose resident lymphocytes and plasma cells provide immune surveillance for the ocular surface. The goblet cells look pale and mucin-filled on haematoxylin and eosin and stain strongly with PAS. Their mucin forms the innermost layer of the tear film, the layer that makes the hydrophobic epithelial surface wettable so the aqueous layer can spread over it instead of beading up.

The conjunctiva stays non-keratinised because the surface must remain moist, and its transitions are precise. Posteriorly it lines the inner surface of the lid as palpebral conjunctiva, reflects at the fornix, and continues over the globe as bulbar conjunctiva. At the limbus it becomes corneal epithelium, and the stem cells that resurface the cornea reside in that limbal zone. At the lid margin, in the other direction, it meets the keratinised epidermis of the outer eyelid, and the change is abrupt.

Trap Do not answer keratinised squamous for the conjunctiva; that is the lid skin, which stops at the lid margin. Simple cuboidal in this region lines the anterior lens surface, the inner face of the tympanic membrane and small gland ducts. Transitional epithelium, or urothelium, lines the urinary tract and occurs nowhere in the eye.

The eyelid and its four glands

A sagittal section of the eyelid is a small tour of integument. The outer surface is thin keratinised stratified squamous epidermis with hair follicles for the lashes; deep to it lies the skeletal orbicularis oculi, supplied by CN VII for lid closure; then the tarsal plate, a sheet of dense collagen that stiffens the lid; and finally the palpebral conjunctiva on the inner surface. Levator palpebrae superioris, a skeletal muscle, raises the lid, and the smooth superior tarsal muscle assists under sympathetic control, which is why a sympathetic lesion produces ptosis.

Four glands occupy the lid, and the organising principle for telling them apart is mode of secretion plus site. Sebaceous glands are holocrine, so at low power they are lobules of large, pale, foamy cells whose entire bodies disintegrate to release oily sebum, with no true lumen of their own. Apocrine sweat glands are coiled tubular structures with a real lumen and myoepithelial cells. Serous glands have tall pyramidal cells with basal nuclei and apical secretory granules.

Gland	Type and location	Secretion and clinical point
Meibomian, or tarsal, glands	Large holocrine sebaceous glands in vertical rows embedded in the dense tarsal plate, opening on the lid margin behind the lashes	Oily meibum, the outermost tear layer. Blockage gives a painless chalazion; poor meibum gives evaporative dry eye
Glands of Zeis	Small holocrine sebaceous glands opening into the eyelash follicles	Contribute lipid at the lash line; infection gives a stye
Glands of Moll	Modified apocrine sweat glands near the lash follicles, coiled tubular with a true lumen and myoepithelial cells	Sweat-like secretion at the lid margin; not sebaceous and not holocrine
Lacrimal gland with accessory glands of Krause and Wolfring	Serous compound tubuloalveolar; the main gland in the superolateral orbit, the accessory glands in the conjunctival fornix	The watery aqueous middle layer of the tear film, with lysozyme and IgA

Trap The gland of Moll is the one lid gland that is not sebaceous. If a prompt asks you to select every sebaceous gland of the eyelid, the answer is meibomian plus Zeis and nothing else. Von Ebner glands are serous glands of the tongue that empty into the trenches around the circumvallate papillae and rinse the taste buds; they have no ocular role at all.

Trap Meibomian glands secrete oil, never water and never mucin, and nothing in the lid secretes aqueous humour. Aqueous humour is intraocular, made by the ciliary epithelium and drained at the trabecular meshwork, and it never reaches the tear film.

The lacrimal gland and the drainage pathway

The lacrimal gland sits in the lacrimal fossa of the frontal bone at the superolateral corner of the orbit, above and lateral to the globe. It is a serous compound tubuloalveolar gland, and a section shows lobules of acini lined by tall pyramidal cells with basal nuclei and apical secretory granules, conspicuous myoepithelial cells wrapped around the acini, unusually wide intralobular ducts, and plasma cells in the interstitium supplying the IgA that is transported into the secretion. Serous means watery, which is exactly what the aqueous middle layer of the tear film requires, and the secretion also carries lysozyme for antibacterial defence.

From the superolateral gland, tears are swept medially across the ocular surface by blinking, and leave the eye at the medial canthus. The sequence is: upper and lower puncta, canaliculi, lacrimal sac, nasolacrimal duct, and finally the nasal cavity, which is why crying makes the nose run.

Rule Secretion is superolateral, drainage is inferomedial. The lower medial corner of the eye holds the puncta, canaliculi and lacrimal sac, and contains no secretory gland of any importance.

Rule Three glands, three modes, three layers. Holocrine sebaceous meibomian oil on the outside, merocrine serous lacrimal water in the middle, goblet-cell mucin on the inside against the epithelium.

Trap The lacrimal gland is serous, not mucous, and it is not inside the tarsal plate. The tarsal plate is dense collagen housing the holocrine meibomian glands, and the apocrine glands of the lid are the glands of Moll at the lid margin. A prompt placing a mucous gland in the lower medial orbit is wrong on type and on site at once.

The tear film as a three-layer structure

Layer, outside in	Source	Role
Lipid	Meibomian glands, holocrine sebaceous, within the tarsal plate	Slows evaporation and stabilises the film between blinks
Aqueous	Lacrimal gland plus accessory glands of Krause and Wolfring	Bulk of the volume; nourishes the cornea, carries lysozyme and IgA, flushes debris
Mucin	Conjunctival goblet cells	Makes the epithelial surface wettable so the aqueous layer can spread

High yield Anterior to the cornea the tear film is its main source of oxygen and part of its nutrition, which is one reason contact lens hypoxia drives corneal neovascularisation. A blocked meibomian duct gives a chalazion, and loss of the lipid layer gives evaporative dry eye even when lacrimal secretion is normal.

6. The outer and middle ear: pinna, meatus, membrane and ossicles

The ear divides into three parts that are structurally unlike one another. The external ear is skin over cartilage and bone, a funnel and a tube. The middle ear is an air-filled space containing a bony lever system and two tiny skeletal muscles, vented to the pharynx. The inner ear is a fluid-filled epithelial labyrinth inside dense bone. The tympanic membrane marks the first boundary and the oval window the second, and sound is converted from air vibration to bone movement to fluid wave as it crosses them.

High yield The external auditory meatus is skin-lined throughout: keratinised stratified squamous epidermis with hair follicles, sebaceous glands and ceruminous apocrine glands, supported by elastic cartilage in the outer third and by temporal bone in the inner two thirds.

The pinna and the cartilage comparison

The pinna, or auricle, is a thin plate of elastic cartilage covered on both surfaces by thin skin carrying hair follicles and sebaceous glands. At the microscope the cartilage shows chondrocytes in lacunae within a matrix crowded with dark, branching elastic fibres, which stand out dramatically with orcein and separate it at once from the clear, glassy matrix of hyaline cartilage. The functional logic is simple: a sound-collecting funnel that projects from the head must hold its shape, yet it must also bend and spring back when knocked or slept on, and elastic cartilage is the only cartilage that does both.

Like all cartilage, the pinna is avascular and is nourished by diffusion from vessels in its perichondrium. Shearing injury that strips the perichondrium away deprives the cartilage of its blood supply and allows a haematoma to form between the two, and the fibrosis and irregular new cartilage that follow produce the deformity known as cauliflower ear.

Cartilage	Matrix appearance	Where it is found
Elastic	Chondrocytes in lacunae in a matrix crowded with dark branching elastic fibres	Pinna, outer third of the external auditory meatus, epiglottis
Hyaline	Glassy, slightly basophilic, homogeneous type II collagen matrix	Costal cartilage, articular surfaces, most of the laryngeal and tracheal skeleton
Fibrocartilage	Dense type I collagen bundles with chondrocytes lined up in rows between them	Intervertebral discs, menisci, pubic symphysis

Trap Bone does occur in the ear, forming the inner two thirds of the external auditory meatus and all three ossicles, but the pinna has to flex, so it is cartilage. Fibrocartilage is built for compression and shear and appears nowhere in the ear.

The external auditory meatus and its two glands

Read the external auditory meatus as a skin-lined tube with a cartilage wall outside and a bone wall inside. The lining is thin keratinised stratified squamous epidermis resting on a dermis, and it carries everything skin carries: hair follicles, sebaceous glands, and apocrine sweat glands modified into the ceruminous glands. The hairs plus the oily, antibacterial cerumen trap dust and insects and keep the canal from macerating.

Distinguishing the two gland types in the dermis at low power is a standard exercise, and the discriminators are size, depth and texture.

Feature	Ceruminous glands	Sebaceous glands
Class and mode	Modified apocrine sweat glands, coiled tubular	Branched acinar and holocrine, attached to hair follicles
Size and depth	Larger profiles lying deeper in the dermis	Smaller and more superficial, beside the follicle
Cells and lumen	Simple cuboidal to columnar cells with pale, lightly eosinophilic cytoplasm and apical blebs, wrapped in myoepithelial cells around an obvious lumen	Solid lobules of large foamy cells that disintegrate entirely, with no lumen of their own

Mixed together at the surface, the apocrine secretion and the sebum constitute cerumen.

Trap Reversing the pair is the usual error. The sebaceous lobules are the smaller, more superficial, follicle-attached glands and the ceruminous coils are the larger, deeper, paler ones. They are in fact easy to separate because they secrete differently: holocrine sebaceous lobules have no lumen, whereas apocrine ceruminous tubules show a clear lumen and myoepithelial cells. Coiled tubular profiles in the meatus are always ceruminous.

Trap There are no goblet cells in the external auditory meatus and no otoliths there either. Goblet cells in the ear occur only at the nasopharyngeal end of the eustachian tube, and otoliths, or otoconia, are calcium carbonate crystals on the maculae of the utricle and saccule deep in the inner ear.

The tympanic membrane: three layers, three germ layers

The tympanic membrane is a three-layer sheet, and its outside-in order is simply the order of its germ layers, because the membrane lies exactly where the first pharyngeal cleft meets the first pharyngeal pouch.

Epidermis, from surface ectoderm Thin keratinised stratified squamous epithelium facing the external auditory meatus, with no hairs and no glands, which is what distinguishes it from the canal skin behind it.
Collagenous core, from mesoderm A thin lamina of collagen with outer radial fibres and inner circular fibres, an arrangement that gives the membrane the stiffness and tension it needs to transmit vibration faithfully rather than damp it.
Simple cuboidal to squamous epithelium, from endoderm The lining facing the middle ear cavity, continuous with the mucosa of the tympanic cavity and the eustachian tube.

Memory hook Outside in, the tympanic membrane is ectoderm, mesoderm, endoderm, in that order. One structure, all three germ layers, because a cleft meets a pouch there.

The ossicular chain

Sound crosses the middle ear from outside in, so the ossicles are encountered in the order malleus, incus, stapes. The handle of the malleus is embedded in the fibrous core of the tympanic membrane, so the membrane and the malleus move together. The head of the malleus articulates with the incus at a synovial joint, the incus passes the vibration on, and the footplate of the stapes sits in the oval window in direct contact with the perilymph of the scala vestibuli.

The chain exists to solve an impedance problem: air is light and compressible, inner ear fluid is dense and incompressible, and a sound wave striking the fluid directly would be almost entirely reflected. The middle ear overcomes this passively in two ways. The ossicles act as a lever system that increases force at the expense of displacement, and the area of the tympanic membrane is far greater than the area of the stapes footplate, so the same total force is concentrated onto a much smaller surface and the pressure rises.

Memory hook Developmental origins follow the sound path in pairs: malleus and incus from the first pharyngeal arch, matching CN V3, and the stapes from the second arch, matching CN VII. Where the bone goes, the nerve goes.

Stapedius and tensor tympani

Two tiny skeletal muscles stiffen the ossicular chain, and the reliable way to keep them apart is by pharyngeal arch and nerve rather than by position.

Feature	Stapedius	Tensor tympani
Attachment	From the pyramidal eminence to the neck of the stapes	Pulls the handle of the malleus medially
Arch and nerve	Second pharyngeal arch, CN VII	First pharyngeal arch, CN V3
Job	Damps loud sound; it is the smallest skeletal muscle in the body	Damps the self-generated noise of chewing

High yield Both muscles attenuate and neither amplifies. A proximal facial nerve lesion paralyses stapedius and produces hyperacusis, which is a useful sign for localising the lesion above the level at which the nerve to stapedius leaves.

Trap Only stapedius belongs to CN VII. Tensor tympani is a first-arch muscle and therefore follows CN V3 along with the muscles of mastication. Swapping the two nerves is the standard error, and the giveaway is arch origin: stapes and stapedius are second arch with CN VII, malleus and tensor tympani are first arch with CN V3.

Trap Amplification in the middle ear is entirely passive, coming from the lever action of the ossicles and from the area ratio of tympanic membrane to stapes footplate. Neither muscle amplifies anything; a prompt that says they do is wrong regardless of which nerve it assigns.

The eustachian tube

The pharyngotympanic, or eustachian, tube runs from the anterior wall of the middle ear cavity downward, forward and medially to the lateral wall of the nasopharynx, where it opens above the soft palate. Its epithelium changes along its length, and the change tracks its two ends. The bony third nearest the middle ear is lined by simple ciliated columnar epithelium. Toward the nasopharynx the lining becomes pseudostratified ciliated columnar with goblet cells, continuous with respiratory mucosa, and this cartilaginous portion is supported by a hook of cartilage with a rich lymphoid infiltrate around it.

The tube is normally collapsed, which protects the middle ear from nasopharyngeal pressure swings and reflux. It is pulled open actively during swallowing and yawning by the tensor veli palatini and levator veli palatini, equalising middle ear pressure with the atmosphere so the tympanic membrane can vibrate freely. The cilia sweep mucus down toward the nasopharynx, clearing the middle ear.

High yield In children the eustachian tube is shorter, wider and more horizontal, so nasopharyngeal organisms reach the middle ear easily and drainage is poor. That single anatomical fact explains why otitis media is a disease of childhood.

Trap The eustachian tube carries air, not endolymph and not nerves. Endolymph is drained through the endolymphatic duct into the endolymphatic sac, and failure of that absorption is linked to Meniere disease. CN VIII runs in the internal acoustic meatus alongside the facial nerve, well away from the middle ear cavity. And the tube opens into the nasopharynx above the soft palate, not into the oral cavity, which is precisely why the palatal muscles are the ones that open it during swallowing.

Rule Follow the epithelium and you can place any point in the first two parts of the ear. Keratinised squamous with hairs and glands means external auditory meatus. Keratinised squamous with no hairs and no glands means the outer face of the tympanic membrane. Simple cuboidal to squamous means its inner face and the middle ear cavity. Ciliated columnar with goblet cells means the pharyngeal end of the eustachian tube.

7. The inner ear: cochlear compartments, organ of Corti and vestibular receptors

The inner ear is an epithelial tube, the membranous labyrinth, suspended inside a bony cavity, and everything about its function turns on the fact that the fluid inside the tube and the fluid outside it have opposite ionic compositions. Outside the membranous labyrinth, in the bony spaces, is perilymph, which is sodium-rich and potassium-poor like any other extracellular fluid. Inside the epithelial tube is endolymph, which is potassium-rich and electrically positive. Sensory hair cells sit with their bodies in the perilymphatic environment and their stereocilia bathed in endolymph, and the resulting electrochemical gradient is the battery that powers transduction.

High yield Perilymph is sodium-rich and fills the scala vestibuli and scala tympani, which communicate at the helicotrema. Endolymph is potassium-rich and fills the scala media, also called the cochlear duct, which houses the organ of Corti.

The three cochlear compartments

The cochlear duct divides the bony spiral canal into three parallel compartments, and the organising question for any one of them is whether it lies inside or outside the epithelial membranous labyrinth.

Compartment	Fluid and boundaries	Key contents
Scala vestibuli	Perilymph; lies above the vestibular membrane, outside the membranous labyrinth	Begins at the oval window where the stapes footplate drives it
Scala media, or cochlear duct	Endolymph; walled off by the vestibular membrane above and the basilar membrane below, lined by epithelium	The organ of Corti on the basilar membrane and the stria vascularis on its lateral wall
Scala tympani	Perilymph; lies below the basilar membrane, outside the membranous labyrinth	Ends at the round window; continuous with the scala vestibuli at the helicotrema

Trap Scala media and cochlear duct are two names for the same compartment, so neither can be perilymphatic. If a prompt asks you to select every perilymph-filled space, the answer is scala vestibuli and scala tympani only.

Memory hook Media sits in the middle and is the odd one out: it is the only compartment inside the epithelium, the only one with endolymph, and the only one with sensory cells.

The stria vascularis and the endocochlear potential

Endolymph is secreted by the stria vascularis, a thickened specialised epithelium on the lateral wall of the scala media, lying over the connective tissue of the spiral ligament. It is the one epithelium in the body with capillaries running inside it rather than only beneath it, and on a radial section it appears as a dark, multilayered band with vessels embedded among its cells. Three cell types are present: marginal cells facing the endolymph, intermediate cells, and basal cells against the spiral ligament.

The marginal cells are the pumps. They are crammed with mitochondria, sodium-potassium ATPase and potassium channels, and they move potassium into the duct. The result is twofold: endolymph becomes potassium-rich, and it becomes electrically positive relative to perilymph. That standing positive voltage is the endocochlear potential, and because hair cell apices are bathed in this positive, potassium-rich fluid while their bases sit in ordinary extracellular fluid, the electrochemical driving force for potassium entry through an opened apical channel is enormous. Transduction is fast and sensitive for exactly this reason.

Trap The choroid plexus is modified ependyma over a fenestrated capillary core in the brain ventricles and secretes cerebrospinal fluid, not endolymph. The organ of Corti transduces sound rather than secreting fluid, and the spiral ganglion in the bony modiolus holds the bipolar first-order sensory neurons whose peripheral processes synapse on the hair cells. Only the stria vascularis secretes.

The organ of Corti

The organ of Corti is the sensory epithelium of hearing. It sits inside the endolymph-filled scala media, resting on the basilar membrane, with the gelatinous tectorial membrane projecting over it from the spiral limbus. A radial section through one turn of the cochlea shows a highly ordered cellular architecture.

One row of inner hair cells Lying medial to the tunnel of Corti, flask-shaped, each contacted by many afferent fibres.
Three rows of outer hair cells Lying lateral to the tunnel, cylindrical, in three to five rows depending on the turn.
Pillar cells Inner and outer pillar cells lean against each other to enclose the triangular inner tunnel, the tunnel of Corti, which is the most conspicuous landmark in the field.
Deiters and Hensen cells Supporting cells that cradle the bases of the outer hair cells and continue laterally across the epithelium.
Tectorial membrane A gelatinous shelf overhanging the hair cells, into which the tallest stereocilia of the outer hair cells are embedded.

The mechanical sequence is straightforward once the geometry is clear. Sound moves the basilar membrane up and down; the tectorial membrane is hinged at a different point, so the two surfaces shear past one another; that shearing deflects the stereocilia, opening mechanically gated channels and letting potassium-rich endolymph into the cell.

Trap The organ of Corti rests on the basilar membrane and lies in the scala media. The tectorial membrane overhangs the hair cells and provides the shearing surface; it never supports them. The scala tympani is the perilymph space below the basilar membrane ending at the round window, and the helicotrema is merely the apical opening where the two perilymphatic scalae meet.

Inner against outer hair cells

Feature	Inner hair cells	Outer hair cells
Arrangement	A single row medial to the tunnel of Corti	Three to five rows lateral to the tunnel of Corti
Innervation	Carry about 95 percent of the afferent synapses to spiral ganglion neurons	Receive mainly efferent olivocochlear fibres that tune the cochlea
Function	Do the actual transduction and transmission of the signal centrally	Change length using the motor protein prestin, amplifying and sharpening the travelling wave
Vulnerability	More robust, lost later	Die first in noise-induced damage and in presbycusis

High yield One inner row transmits and three outer rows amplify. Because outer hair cells are the first to go in noise exposure and in age-related loss, early sensorineural hearing loss is a loss of amplification and frequency sharpening before it is a loss of raw transduction.

Trap Reversing the two is the standard error. Amplification by length change is unique to the outer hair cells, which alone contain prestin, and almost all afferent fibres to the spiral ganglion arise from inner hair cells. They are not both afferent and not both amplifiers.

The hydraulic path of sound and the tonotopic map

Perilymph cannot be compressed, so a wave driven into a closed bony box has to have somewhere to go. The cochlea solves this with two windows and a connecting hole at the apex.

Oval window The stapes footplate pushes the perilymph of the scala vestibuli inward.
Scala vestibuli The pressure wave travels from base toward apex.
Helicotrema At the apex the two perilymphatic scalae communicate, so the wave passes across.
Scala tympani The wave returns from apex toward base.
Round window Its membrane bulges outward into the middle ear as the pressure relief valve, allowing the whole column to move.

The travelling wave set up by this loop deflects the basilar membrane and, with it, the scala media sandwiched between the two perilymphatic spaces, so the hair cells are sheared against the tectorial membrane at whichever point along the spiral the wave peaks. Where that peak falls depends on the mechanics of the membrane itself: the basilar membrane is narrow and stiff at the base and wide and floppy at the apex, so high frequencies peak near the base and low frequencies near the apex. That gradient is the tonotopic map, and it is preserved all the way into the auditory cortex.

Memory hook Base is stiff, narrow, high pitch. Apex is floppy, wide, low pitch. A tight short string sounds high and a long slack one sounds low, exactly as on any stringed instrument.

Rule The oval window is the input and the round window is the output, and both are required. Fixation of the stapes footplate at the oval window, as in otosclerosis, blocks the input and gives conductive hearing loss even though the cochlea itself is intact.

Hair cell transduction in detail

Every hair cell in the inner ear, cochlear or vestibular, carries an apical bundle of actin-filled stereocilia arranged as a staircase from short to tall, with a single true kinocilium at the tall edge in the vestibular receptors. Neighbouring stereocilia are joined tip to shaft by fine tip links, so the whole bundle moves as a unit and tension is shared across it.

Deflection toward the kinocilium, or toward the tall edge The tip links are stretched, which pulls open mechanically gated cation channels at the tips.
Potassium entry Because the endolymph bathing the apex is potassium-rich and positive, potassium flows into the cell and depolarises it.
Transmitter release Depolarisation opens voltage-gated calcium channels at the base and more glutamate is released onto the afferent ending.
Firing rate rises The afferent fibre increases its discharge above its resting rate.
Deflection away from the kinocilium The tip links slacken, the channels shut, the cell hyperpolarises, and the afferent firing rate falls below its resting rate.

High yield Hair cells have a tonic resting discharge, and movement pushes that rate up or down. Swinging either side of a baseline is how a single cell encodes the direction of a stimulus, not just its presence.

Trap A silent, unchanged response to deflection would make direction coding impossible, and bending away from the kinocilium does not do nothing: it closes channels and drops firing below the resting rate. The cupula, meanwhile, stays anchored to the crista and to the hair cell bundles throughout; it bends like a swinging door rather than coming free.

The vestibular receptors: maculae and cristae

Both vestibular receptor types are patches of type I and type II hair cells with supporting cells, and the structure that separates them is the accessory gel sitting on top. That gel determines which physical stimulus the patch can detect.

Feature	Macula	Crista ampullaris
Location	A flat plaque in the utricle and in the saccule	A ridge in the ampulla of each semicircular canal
Accessory structure	The otolithic membrane, a gel loaded with dense calcium carbonate otoliths, or otoconia	The cupula, a tall gel with no otoliths, matched in density to endolymph
Stimulus detected	Linear acceleration and static head tilt, because the dense otoliths lag behind by inertia and drag the gel	Angular acceleration, that is rotation, because endolymph lags and swings the cupula

Rule Otoliths mean gravity and straight-line movement; a density-matched cupula means rotation. Because the cupula has the same density as the fluid around it, gravity acts on both equally and it is unresponsive to tilt, which is exactly what a rotation sensor needs.

Trap Swapping the gels is the classic mistake: the cupula is the otolith-free gel of the crista and otoliths sit only on the maculae. Neither receptor senses sound; sound is transduced only by the organ of Corti in the cochlea, while the vestibular receptors report head position and head movement.

High yield Otolith debris that breaks loose and drifts into a semicircular canal makes the cupula respond to gravity as well as to rotation, producing brief positional vertigo. That is benign paroxysmal positional vertigo, and it is a mechanical fault, not a degeneration of hair cells.

Finally, endolymph is not simply secreted and retained. It is reabsorbed through the endolymphatic duct into the endolymphatic sac, and the first-order afferents from all these receptors run centrally in CN VIII, which travels in the internal acoustic meatus alongside the facial nerve. The cochlear afferents have their cell bodies in the spiral ganglion within the bony modiolus, the central pillar around which the cochlear spiral is wound.

8. Clinical correlation: every disease sits in a named layer

The payoff of this histology is diagnostic localisation. Almost every condition of the eye and ear in common use as an examination item can be assigned to a single named layer, cell type or membrane, and once it is assigned, the clinical picture follows from what that structure was doing. This closing section works through the correlations in the same order as the preceding sections, so each disease lands next to the histology that explains it.

High yield Match the lesion to the layer: Fuchs dystrophy is corneal endothelium, glaucoma is the trabecular meshwork and canal of Schlemm, cataract is lens crystallins, wet macular degeneration is Bruch's membrane, retinal detachment is the pigment epithelium interface, otosclerosis is the stapes footplate, and presbycusis is the outer hair cells.

Cornea: the price of avascularity and a non-dividing endothelium

Three corneal properties generate three distinct clinical consequences. The dense trigeminal innervation makes a trivial epithelial abrasion disproportionately painful, because the free nerve endings of the ophthalmic division sit immediately beneath the surface cells. The avascularity confers immune privilege, so grafts take without HLA matching, but it also means the cornea depends entirely on diffusion from the tear film, the aqueous humour and the limbal capillaries, and chronic hypoxia from contact lens wear drives new vessels in from the limbus. Those vessels cloud the stroma directly and, by abolishing the immune privilege that depended on having no vessels and no lymphatics, raise the risk of rejection in any subsequent graft.

The third property is that the endothelium does not divide in adults. Losses from age, from surgery or from a primary dystrophy are covered only by neighbouring cells spreading thinner, so endothelial density falls irreversibly. In Fuchs endothelial dystrophy, nodular excrescences of Descemet's membrane called guttae appear while the endothelial cells above them are lost; once the remaining pump cannot keep up with the leak, the stroma imbibes water, the collagen lattice spacing is disturbed, and the cornea develops the hazy oedema that is the hallmark of the disease.

Two further correlations belong here. Keratinisation of the corneal epithelium, as occurs in vitamin A deficiency, clouds the cornea because keratin scatters light, and it represents a surface epithelium losing its non-keratinised phenotype. And any injury that cuts through Bowman's membrane heals with a scar, because Bowman's does not regenerate, whereas an injury confined to the epithelium heals clear from limbal stem cells within days.

Trap Corneal oedema and cataract both present as a cloudy anterior eye but they are in different structures. Cloudiness arising from failed endothelial pumping is corneal; cloudiness arising from aggregated crystallins in anucleate fibres is lenticular. The distinguishing histology is stromal fibril spacing against lens fibre packing.

Uvea and the aqueous apparatus

Condition	Structure involved	Why the histology explains it
Uveitis	Choroid, ciliary body and iris together	All three are the same vascular, pigmented connective tissue with a shared blood supply, so inflammation rarely respects their boundaries
Uveal melanoma	Resident melanocytes of choroid, ciliary body or iris	The uvea is densely populated with melanocytes, which are the cell of origin
Glaucoma	Trabecular meshwork and canal of Schlemm at the iridocorneal angle	Aqueous is secreted continuously, so obstruction of the only outflow route raises pressure and damages the optic nerve head
Horner syndrome	Dilator pupillae and superior tarsal muscle, both sympathetically driven smooth muscle	Losing the radial dilator leaves the parasympathetic circular sphincter unopposed, giving miosis, and losing the tarsal muscle gives ptosis

Two pharmacological correlations follow directly from the epithelium and the muscle. Carbonic anhydrase inhibitors lower intraocular pressure because the non-pigmented ciliary epithelium depends on carbonic anhydrase to secrete aqueous humour, so the drug acts on production rather than on drainage. Antimuscarinic drugs cause mydriasis by blocking the parasympathetic sphincter pupillae and cycloplegia by blocking the parasympathetic ciliary muscle, so the zonule stays taut and the lens cannot round up.

Presbyopia is the one condition in this group whose lesion is not in the uvea at all. The ciliary muscle continues to contract and the zonule continues to slacken, but the lens has stiffened with the lifelong accumulation of fibres, so it no longer rounds up when released. The structure at fault is lens elasticity.

Trap Albinism produces a pink iris because no melanin is made anywhere, so choroidal and iridal vessels show through. It is not a vascular disease and it is not a disease of the pigment epithelium being absent; the two optic-cup layers are present, they simply contain no melanin.

Retina and choroid: barriers that fail

The outer retina has no vessels of its own and lives on diffusion from the choriocapillaris across Bruch's membrane, with the tight junctions of the pigment epithelium as the seal. Every major retinal disease in this set is a failure somewhere along that chain.

Dry age-related macular degeneration Metabolic debris accumulates as drusen at the level of Bruch's membrane, between the choriocapillaris and the pigment epithelium, degrading the diffusion path on which the photoreceptors depend.
Wet age-related macular degeneration New vessels grow from the choriocapillaris through Bruch's membrane into the subretinal space, leaking and bleeding, and central vision fails abruptly because the macula is affected.
Retinal detachment The neural retina separates from the pigment epithelium along the obliterated intraretinal space of the optic cup. Cut off from the choriocapillaris and from retinal recycling, the detached photoreceptors die, which is why timing matters clinically.
Posterior vitreous detachment The vitreous separates from the inner retinal surface, producing floaters, and traction at points of firm adhesion can tear the retina and allow fluid into the subretinal plane.
Papilledema Raised intracranial pressure obstructs flow in the ganglion cell axons as they turn through the sclera at the optic disc, so the disc swells. Only the disc can do this, because only there do unmyelinated retinal axons become a myelinated nerve crossing a pressure boundary.
Choroidal detachment Fluid or blood fills the potential suprachoroidal space between choroid and sclera, a completely different plane from retinal detachment.

High yield Both forms of macular degeneration are centred on Bruch's membrane, and that is also the structure the retinal pigment epithelium sits on and the diffusion barrier of the outer blood-retina barrier. Naming the membrane names the disease.

Trap The physiological blind spot at the optic disc is normal and is caused by absent photoreceptors, not by any pathology and not by a lack of blood supply. The avascular region of the retina is the fovea, and that avascularity is a feature that improves acuity rather than a defect.

Lens, lids and tear film

The lens generates two kinds of disease, one from its proteins and one from its suspension. Cataract is the protein disease: crystallins in fibres that are never replaced accumulate oxidative and glycation damage, cross-link, aggregate into Morgagnian globules and scatter light, with ageing, diabetes, corticosteroids, smoking and ultraviolet exposure as the predictable risks. Lens subluxation is the suspension disease: because the zonule is built of fibrillin-rich oxytalan microfibrils, fibrillin disorders loosen it. In Marfan syndrome the lens dislocates upward and in homocystinuria downward, a pairing worth memorising as a unit because the direction is the discriminator.

On the lid, the diseases follow the glands. A blocked meibomian duct within the tarsal plate traps holocrine sebum and produces a chalazion, a painless lid nodule. Infection of the smaller sebaceous glands of Zeis at a lash follicle produces a stye at the lid margin. Loss of meibomian oil leaves the aqueous layer unprotected, so it evaporates between blinks and the result is evaporative dry eye even when the lacrimal gland is working normally. Deficient conjunctival goblet cells cause a different kind of dry eye, in which the surface is not wettable and the aqueous layer will not spread at all.

Trap Dry eye is not one disease. Aqueous-deficient dry eye is a lacrimal gland problem, evaporative dry eye is a meibomian problem, and failure of surface wetting is a goblet cell problem. The tear film has three layers and three sources, so it can fail in three ways.

Ear: conduction against transduction

Condition	Structure involved	Mechanism from the histology
Cauliflower ear	Perichondrium of the auricular elastic cartilage	Cartilage is avascular and fed through its perichondrium, so stripping it starves the cartilage and the healing response deforms the pinna
Otitis media in children	Eustachian tube	Short, wide and horizontal in childhood, so nasopharyngeal organisms ascend easily and the middle ear drains poorly
Otosclerosis	Stapes footplate at the oval window	A fixed footplate cannot drive the perilymph of the scala vestibuli, so the conductive input is lost while the cochlea remains normal
Hyperacusis	Stapedius, supplied by CN VII	A proximal facial nerve lesion paralyses the muscle that damps loud sound, so ordinary sounds become intolerable
Noise-induced loss and presbycusis	Outer hair cells and their prestin-based amplification	Outer hair cells die first, so amplification and frequency sharpening are lost before raw transduction by the inner row
Benign paroxysmal positional vertigo	Otoconia displaced into a semicircular canal	Dense crystals in a canal make the normally density-matched cupula respond to gravity, so head position triggers brief vertigo
Meniere disease	Endolymph reabsorption at the endolymphatic duct and sac	Failure of absorption distends the endolymphatic compartment, disturbing both cochlear and vestibular function

The conduction against transduction distinction is the organising clinical idea for the ear, and it maps exactly onto the histology already covered. Anything from the pinna through the external auditory meatus, the tympanic membrane, the ossicular chain and the oval window is a conducting apparatus made of skin, cartilage, collagen and bone. Anything from the perilymph of the scala vestibuli inward, including the stria vascularis, the basilar membrane, the hair cells and the spiral ganglion, is a transducing apparatus built of specialised epithelium and neurons. Impacted cerumen, a perforated membrane and a fixed stapes are all conductive; stria vascularis failure, hair cell loss and spiral ganglion disease are all sensorineural.

High yield Both middle ear muscles attenuate rather than amplify, and the actual amplification in the middle ear is passive, from ossicular lever action and from the area ratio of tympanic membrane to stapes footplate. A lesion of either muscle therefore causes intolerance of loud sound, not deafness.

Trap Tonotopy makes the site of cochlear damage predictable, so do not treat hearing loss as uniform across frequencies. The base of the cochlea is stiff and narrow and answers high frequencies, and it is the base that bears the brunt of noise exposure and ageing, which is why high-frequency loss appears first.

Rule For every clinical item in this region, name the layer before you name the disease. Epithelium, membrane, stroma, endothelium, pigment epithelium, hair cell, footplate: the structure tells you both the symptom and the reason the symptom has the character it does.
