1. Orientation and the master timeline

The pharyngeal apparatus is a construction project with a fixed order: build the scaffold, populate it with neural crest, wire it with a nerve and an artery, then demolish the temporary parts. Each arch is an independent module that owns one cartilage bar, one muscle group, one nerve and one artery, and that modularity is what makes the whole topic predictable. Get the timeline first, because almost every "when" question and every teratogen timing question is answered from it.

Weeks 3-3.5 Arches 1 and 2 are present. The embryo has just gained its three definitive germ layers; the pericardial bulge is forming and the neuropores are still open. An arch at this stage is only a bar of mesoderm covered outside by surface ectoderm and lined inside by endoderm. No neural crest yet.
Weeks 4-4.25 Arch 2 (the hyoid arch) is distinct as its own arch, within a quarter of a week of arch 1. Arch 1 divides into a maxillary process and a mandibular process.
Weeks 4.5-5 Neural crest migrates in, mainly from hindbrain rhombomeres with a substantial midbrain contribution and only a minor forebrain contribution. The cells travel in discrete streams and the rhombomere of origin fixes the destination arch.
Week 5 Arches 3, 4 and 6 become visible, about a week and a half behind arches 1 and 2, because arches are added in a strict rostral to caudal wave. Pouches are well defined.
Weeks 5-6 Pouch 1 elongates into the tubotympanic recess.
Weeks 5.5-6 Arches, clefts and pouches are all formed, and each arch acquires its own nerve and its own aortic arch artery. Once assigned, the nerve stays with its muscle wherever that muscle migrates.
Weeks 6-7 Arches are mature and fusing; arch 2 is overgrowing clefts 2 to 4 to create the cervical sinus.
Week 8 Meckel's cartilage regresses, having already templated the ossicles. The cervical sinus closes.
Week 10 The pouch 3 and 4 glands sever their pharyngeal connections and finish migrating: thymus to the midline mediastinum towing the inferior parathyroid, superior parathyroid onto the dorsal thyroid.
Month 4 The centre of the pouch 2 bud apoptoses to hollow out the tonsillar crypt. Lymphoid seeding is incomplete until the last months of gestation.

Rule Arches are added rostral to caudal, and the series is always 1, 2, 3, 4, 6. Arch 5 either never forms in humans or forms transiently and regresses completely, leaving no cartilage, no muscle, no nerve and no adult artery. An answer option naming arch 5 is a free elimination.

Memory hook Scaffold at week 3-4, crest at 4.5-5, caudal arches at 5, nerve plus artery at 5.5-6, Meckel's gone at 8, glands land at 10, crypts hollow at month 4.

2. What an arch is made of

An arch is a sandwich with a filling: three germ layers contributing four cell populations, each with a fixed job. Crest is the fourth population and is itself ectoderm derived, arriving only after the sandwich already exists.

Population	Germ layer	Where it sits	What it builds
Surface ectoderm	Ectoderm	Covers the outside; lines the clefts	Cleft linings and skin; external auditory meatus; outer layer of the tympanic membrane
Endoderm	Endoderm	Lines the pharyngeal inside; lines the pouches	All the pouch glands: middle ear and eustachian tube, palatine tonsil, thymus, parathyroids, thyroid C cells. Never makes cartilage.
Mesoderm (mesenchyme): paraxial plus lateral plate	Mesoderm	Fills the arch core; one separate pool per arch	Muscle of the face and neck; arch artery endothelium. Lateral plate additionally builds the arch 4 and 6 laryngeal cartilages.
Neural crest	Ectoderm derived, migratory	Migrates into the arch core at weeks 4.5-5	The skeletal and connective tissue component of the face: cartilage bars, membrane bones of jaw and midface, ligaments, dermis. Also arch artery smooth muscle, and it helps septate the cardiac outflow tract.

Memory hook CAP for the geometry: Clefts are ectoderm, Arches are mesoderm, Pouches are endoderm. Pouches are inside, clefts are outside.

Rule The side you are on tells you the germ layer. A pouch is the inward indentation lined by endoderm, numbered 1 to 4, and because it is endoderm it makes glands. A cleft (groove) is the matching outward indentation lined by surface ectoderm, so it makes ectodermal structures and ectodermal pathology.

Rule Crest is predetermined for skeleton, mesoderm for muscle, endoderm for glands. This division of labour is why a pure crest disorder wrecks the jaw and midface while leaving the pouch glands intact, and why a pouch disorder gives endocrine and immune failure with a near-normal jaw.

Trap Notochord and trophoblast never enter an arch. The notochord is a midline axial rod that induces the neural tube and persists as the nucleus pulposus; trophoblast is extraembryonic and builds the placenta. Neither is an arch population.

Trap Intermediate mesoderm (kidneys, gonads, urogenital system) has no arch role, and "splanchnic mesoderm only" is wrong because it is just one layer of lateral plate and ignores the paraxial contribution. The arch core is paraxial plus lateral plate.

3. The master arch table

This is the single object to be able to reproduce from memory. Every arch owns one cartilage, one muscle group, one nerve and one artery, and the nerve follows the muscle however far it migrates.

Arch	Cartilage	Skeletal derivatives	Muscles	Nerve	Artery
1 (mandibular; chewing and ossicles)	Meckel's cartilage (neural crest). Template only; regresses week 8.	Maxillary process, membrane bone: premaxilla, maxilla, zygomatic, part of temporal. Mandibular process: mandible (membrane bone from crest mesenchyme around Meckel's, from about week 6); malleus and incus (dorsal Meckel's remnants, endochondral); sphenomandibular ligament.	Temporalis, masseter, medial and lateral pterygoids (the 4 chewers), mylohyoid, anterior belly of digastric, tensor tympani, tensor veli palatini.	CN V3 (mandibular division of trigeminal)	Not covered in this set
2 (hyoid; facial expression)	Reichert's cartilage (neural crest)	Stapes, styloid process, stylohyoid ligament, lesser horn and upper body of hyoid.	Muscles of facial expression, auricular muscles, stapedius, stylohyoid, posterior belly of digastric.	CN VII (facial)	Not covered in this set
3 (swallowing)	Its own crest-derived bar	Greater horn and lower body of hyoid. No laryngeal cartilage.	Stylopharyngeus only (elevates the pharynx in swallowing).	CN IX (glossopharyngeal)	Internal carotid / the carotids
4 (larynx and pharynx)	Thyroid cartilage (lateral plate mesoderm)	Thyroid cartilage	Cricothyroid, levator veli palatini and the other palatal elevators, pharyngeal constrictors.	CN X via the superior laryngeal nerve (external branch to cricothyroid); pharyngeal branch to constrictors and palate.	Aortic arch; right subclavian artery
5	None	None	None	None	None
6 (larynx)	Cricoid, arytenoid, corniculate, cuneiform (lateral plate mesoderm)	Those four laryngeal cartilages	All the other intrinsic laryngeal muscles (every intrinsic muscle except cricothyroid).	CN X via the recurrent laryngeal nerve	Pulmonary arteries; ductus arteriosus

Rule One arch, one nerve. Every muscle sharing an arch shares that arch's nerve, so a muscle set is defined by its nerve rather than by its final location. That is why V3 reaches the middle ear (tensor tympani) and the soft palate (tensor veli palatini), and why CN VII reaches the scalp and the middle ear.

Memory hook Nerve ladder: 1 = V3, 2 = VII, 3 = IX, 4 and 6 = X, with arch 4 taking superior laryngeal and arch 6 taking recurrent laryngeal.
Cartilage hook: Meckel = Mandibular = arch 1; Reichert = aRch 2.
Arch 2 is the S arch: Stapes, Styloid process, Stylohyoid ligament, plus lesser horn and upper hyoid body.
Ossicles run 1-1-2: malleus (1), incus (1), stapes (2).

High yield Arches 1, 2 and 3 build their cartilage and facial skeleton from neural crest. Arches 4 and 6 build the laryngeal cartilages from lateral plate mesoderm, with little or no crest. Only the skeletal germ layer changes; the arch core mesoderm still supplies the muscle and the vagus still innervates it. This is why a pure crest disorder such as Treacher Collins spares the larynx, while 22q11.2 deletion disturbs arch 4 and gives aortic arch anomalies.

High yield The hyoid is the bone shared between two arches, and the split is a clean upper versus lower rule: arch 2 takes the lesser horn and upper body, arch 3 takes the greater horn and lower body. The arch with the higher nerve takes the higher part of the bone.

The twin traps: look-alike muscles in different arches

Pair	Arch 1 member (CN V3)	Other member
Digastric	Anterior belly	Posterior belly: arch 2, CN VII. One muscle carries two nerves across its intermediate tendon.
Middle ear muscles	Tensor tympani (on malleus, arch 1)	Stapedius: arch 2, CN VII, on the stapes (arch 2).
Veli palatini	Tensor veli palatini, the only palatal muscle NOT supplied by the vagus	Levator veli palatini: arch 4, vagus via the pharyngeal plexus.

Trap "Stylo-" names span three arches and three nerves. Stylohyoid is arch 2, CN VII. Stylopharyngeus is arch 3, CN IX, and is the only muscle in the body with CN IX motor supply. Styloglossus is not an arch muscle at all: tongue muscles come from occipital somites and are supplied by CN XII. Shared origin on the styloid process tells you nothing about the arch.

Trap CN XII is not an arch nerve. CN V2 is not an arch nerve either: it carries sensation from maxillary-process skin and mucosa but no branchial motor fibres, because the branchial motor nerve of arch 1 is always V3.

Trap Arch 4 and arch 6 both use the vagus, and the branches are the favourite swap. Arch 4 = superior laryngeal (cricothyroid). Arch 6 = recurrent laryngeal (everything else). The recurrent course is itself an arch artery story: the nerve hooks under the arch 6 derived ductus arteriosus on the left and the arch 4 derived subclavian on the right.

Reading a lesion backwards to its arch

Clinical cluster	Arch and nerve	Why those deficits travel together
Weak chewing, jaw deviation, slack eardrum	Arch 1, CN V3	Chewers plus tensor tympani share V3.
Cannot wrinkle the forehead (frontalis) plus hyperacusis (stapedius)	Arch 2, CN VII	Two apparently unrelated muscles, one arch, one nerve. Hyperacusis is the giveaway.
Lost gag reflex, swallowing difficulty	Arch 3, CN IX	CN IX is the afferent limb of the gag reflex; the vagus is the efferent limb.
Weak high voice, palatal droop	Arch 4, superior laryngeal nerve	Cricothyroid plus palatal elevators.
Hoarseness	Arch 6, recurrent laryngeal nerve	All other intrinsic laryngeals.
Dysphagia, nasal regurgitation and hoarseness together	Arches 4 and 6, vagus	One nerve covers both caudal arches.

CN IX has a tiny motor share but a large sensory territory that maps exactly onto arch 3 and its neighbouring pouches: posterior third of the tongue, oropharynx, the palatine tonsil region (pouch 2), and the middle ear via the tympanic branch (pouch 1 derivatives). Sensation follows the arch just as motor supply does.

4. Ossification: membrane versus endochondral

Arch 1 is the whole lesson. Meckel's cartilage is a scaffold, not raw material: the crest mesenchyme condensing around it ossifies directly in membrane to build the jaw, while only the two dorsal remnants of the cartilage itself are replaced by bone.

Derivative	Process	Source
Premaxilla, maxilla, zygomatic, part of temporal	Membrane (intramembranous): mesenchyme ossifies directly, no cartilage template	Maxillary-process neural crest
Mandible	Membrane	Crest mesenchyme condensing lateral to Meckel's cartilage from about week 6
Malleus and incus	Endochondral: cartilage laid down first, then replaced by bone	The two dorsal, most proximal remnants of Meckel's cartilage, the only arch 1 bones using a cartilage precursor
Sphenomandibular ligament	Persisting fibrous remnant	Meckel's cartilage
Stapes, styloid process	Endochondral	Reichert's cartilage (arch 2)

Rule In arch 1, the cartilage bar templates the ossicles while the surrounding crest mesenchyme builds the jaw and midface. This is exactly why a TCOF1 (Tcof-1) defect in arch 1 crest gives mandibular hypoplasia even though Meckel's cartilage itself can form, and why Treacher Collins flattens zygoma and mandible together and also deforms the middle ear.

Trap "Meckel's cartilage ossifies into the mandible" is wrong. Meckel's never becomes the mandible; it is a transient template that regresses by about week 8. Equally, "malleus and incus by membrane ossification" is the right bones with the wrong process.

5. The pouch table

Pouches are endodermal evaginations numbered cranial to caudal, forming in weeks 4-5. Being endoderm is why they make glands.

Pouch	Dorsal derivative	Ventral derivative	Detail to hold
1	Dorsal part elongates at weeks 5-6 into the tubotympanic recess. The expanded distal end becomes the tympanic cavity (middle ear); the narrow proximal stalk keeps the pharyngeal connection as the eustachian (auditory) tube.	No separate ventral derivative	The only pouch that contacts a cleft. Middle ear is endoderm lined, drains to the pharynx, and is served sensorily by the tympanic branch of CN IX. A child's cold ascends the tube to cause otitis media. Mastoid air cells are a later postnatal pneumatization of the tympanic cavity, not a derivative of the whole recess.
2	Palatine tonsil (whole pouch, no dorsal-ventral split): solid endodermal buds grow into crest mesenchyme. Endoderm becomes the surface and crypt lining; mesenchyme supplies the stroma.	No separate ventral derivative	By about month 4 the centre of the bud apoptoses, hollowing it into a blind-ended tonsillar crypt. Lymphocytes arrive later and seeding is incomplete until the last months of gestation, so neonatal tonsils are small and hypertrophy shows only in early childhood. Crypts explain tonsilloliths and the debris trapping behind recurrent tonsillitis. The tonsil sits in the lateral oropharyngeal wall between the palatoglossal and palatopharyngeal folds, which are arch 4 and 6 muscles.
3	INFERIOR parathyroid	Thymus	Thymic endoderm forms the epithelium; incoming haematopoietic cells supply the lymphocytes and crest the capsule and septa. The bilateral primordia lose their pharyngeal connection, migrate caudally and medially and fuse in the midline behind the sternum by week 10; the gland grows until puberty then involutes, so a large thymic shadow is normal in a child.
4	SUPERIOR parathyroid	Ultimobranchial body (also called pouch 5) -> thyroid parafollicular C cells -> calcitonin	The superior parathyroid loses its pharyngeal connection, attaches to the posterior surface of the descending thyroid and settles near the cricothyroid junction by week 10, which is why its adult position is remarkably constant. The ultimobranchial body detaches, is invaded by neural crest and is incorporated into the descending thyroid.

Migration and the pouch 3 / pouch 4 crossover

Rule Pouch 3 starts higher than pouch 4 but finishes lower. The thymus (pouch 3 ventral) migrates caudally and medially into the mediastinum, and the dorsal wing of the same pouch, the inferior parathyroid, stays tethered and is dragged along. Pouch 4's dorsal wing has no passenger role, because its ventral partner, the ultimobranchial body, has already detached; so the superior parathyroid travels only as far as the descending thyroid. All of this is finished by week 10.

Memory hook Pouches 1 to 4 in order: Ear, Tonsils, Bottom, Top. Bottom = 3 (inferior parathyroid plus thymus), Top = 4 (superior parathyroid). Then add the mechanism: the thymus tows the pouch 3 gland down past pouch 4's.

High yield A parathyroid adenoma found low in the neck, in the anterior mediastinum or inside the thymus is almost always a pouch 3 gland, because that is the gland with the long migration.

High yield Two lineages in one thyroid. Follicular cells making T4 and thyroglobulin come from the midline thyroid diverticulum that descends from the foramen cecum of the tongue. C cells between the follicles come from the ultimobranchial body of pouch 4 or 5. That split explains why medullary thyroid carcinoma secretes calcitonin and travels in MEN2 with pheochromocytoma.

Trap Chief cells secreting PTH come from the dorsal wings of pouches 3 and 4. Thymic epithelium is pouch 3 ventral. The ultimobranchial body is pouch 4 ventral. Mixing the dorsal and ventral wings of the same pouch is the most common error here, and the phrase "forms two glands" describes pouch 3 specifically.

Trap Pouch timings are distinct. Pouches and arches form in weeks 4-5; the tubotympanic recess elongates at weeks 5-6; gland migration is done by week 10; tonsillar crypt cavitation is month 4. Week 7 falls in the middle of gland migration, with the thymic primordia still descending.

6. The cleft table

Clefts, or grooves, are ectodermal indentations on the outside of the neck between successive arches. Only one of them yields a definitive adult structure; the rest are supposed to disappear, so their persistence is pathological.

Cleft	Fate	Consequence
1	The external auditory meatus, the only definitive structure formed by any cleft. Its ectoderm deepens toward pouch 1, forms a solid meatal plug that later recanalizes.	Contributes the outer layer of the tympanic membrane where it abuts pouch 1 endoderm. The ear canal is lined by skin because it is ectoderm.
2, 3, 4 (overgrowth covers clefts 2 to 5)	Arch 2 proliferates rapidly and its ectoderm grows caudally like a flap over clefts 3 to 5, then fuses with the epicardial ridge, the surface ectoderm elevation over the developing heart. That fusion seals the clefts inside a smooth ectoderm-lined cavity, the cervical sinus, which normally obliterates by about week 8 and leaves a clean neck contour.	If the cervical sinus persists, the trapped epithelium keeps secreting: lateral cervical (branchial cleft) cyst, with or without a fistula. Arch 2 is the dominant overgrower, so cleft 2 is the one whose failed sealing creates lateral neck pathology.

High yield The tympanic membrane is the one site in the head where an outside cleft and an inside pouch are directly apposed. Three layers: cleft 1 ectoderm outside, a thin fibrous middle layer of crest and mesoderm, pouch 1 endoderm lining the middle ear. Pouch 1 is the exception that proves the clefts-outside, pouches-inside rule. The two epithelia meet but never swap tissue, so the meatus is never endoderm and the tympanic cavity is never ectoderm.

Rule Use the words precisely: cyst = no opening, sinus = one opening, fistula = two openings. A branchial fistula is a patent epithelial tract joining the inside of the neck to the skin, created when a cervical sinus persists AND keeps both openings. The classic second arch form runs from the tonsillar fossa (pouch 2, endoderm), between the internal and external carotid arteries, to a pinpoint skin opening at the anterior border of the sternocleidomastoid (cleft 2, ectoderm), so it literally joins a pouch to a cleft. Intermittent saliva-like drainage is the clue; treatment is complete excision.

Neck masses: lateral versus midline

Mass	Origin	Location and bedside sign
Branchial cleft cyst	Retained ectodermal lining of the cervical sinus along the path of cleft 2	Lateral neck, just anterior to the sternocleidomastoid near the mandibular angle. Smooth, painless, squamous lined. Does not move on swallowing or tongue protrusion. Usually appears in late childhood and enlarges after an upper respiratory infection.
Thyroglossal duct cyst	Remnant of thyroid descent from the foramen cecum	Midline, below the hyoid; elevates on tongue protrusion or swallowing.
Lingual thyroid	Ectopic thyroid tissue that never descended	Base of the tongue near the foramen cecum.
Cystic hygroma	Congenital lymphatic malformation from failed connection of the jugular lymph sacs	Soft mass in the posterior triangle, behind the sternocleidomastoid. Linked to Turner and Down syndromes.
Dermoid cyst	Inclusion of surface ectoderm along a fusion line	Contains skin adnexal structures such as hair follicles and sebaceous glands.

Memory hook Lateral equals cleft (ectoderm). Midline equals failed descent of an endodermal diverticulum.

Trap Several midline pharyngeal landmarks are offered as decoys for cleft and pouch questions. The foramen cecum is the midline tongue pit at the junction of anterior two thirds and posterior third, where the thyroid diverticulum began its descent. The hypobranchial eminence is a midline swelling of arch 3 and 4 mesoderm in the pharyngeal floor that forms the posterior third of the tongue, and the epiglottis grows from it rather than from any arch cartilage bar. The tuberculum impar is the midline arch 1 swelling overgrown by the lateral lingual swellings to build the anterior two thirds of the tongue. None of these is a cleft, a pouch or the epicardial ridge.

7. Hox code and rhombomeres

Hox proteins are transcription factors expressed in hindbrain neuroectoderm. They reach the arches because neural crest cells delaminate from specific rhombomeres and migrate ventrolaterally carrying that expression with them. Each stream arrives pre-labelled with the identity of the rhombomere it left, and that label tells the arch mesenchyme which cartilage bar, bones and connective tissue to build. Arches are therefore patterned from the crest inward, not by endoderm and not by vessels.

Arch	Crest donor	Coding factor	Matching feature
1	R1, R2 plus midbrain	OTX2; Hox-free	Maxillary and mandibular skeleton, malleus and incus, trigeminal innervation
2	R4 only	Hox coded	Reichert's cartilage, stapes, styloid, upper hyoid; the same hindbrain level houses the CN VII nucleus
3	R6, R7	Hoxa-3 prominent	Greater horn of hyoid, stylopharyngeus; the level of the CN IX nucleus
4 and 6	R8	Caudal Hox	Vagal, laryngeal territory. R8 crest contributes connective tissue and patterning information but does not build the cartilage bars, which are lateral plate mesoderm.

R3 and R5 contribute very little crest and are largely crest free, so no arch takes its identity from them.

Memory hook The even donors climb with the arches: R2, R4, R6, R8 for arches 1, 2, 3, 4/6. The absence of a separate donor segment also fits the absence of a functional arch 5.

High yield Arch 1 is the Hox-free arch. It is patterned instead by OTX2, expressed in forebrain and midbrain derived crest and in the R1-R2 stream. Hox coding starts caudally, with Hoxa-3 for arch 3 and beyond.

Rule Hox genes are expressed in overlapping domains along the rostrocaudal axis: 3 prime genes switch on earlier and more rostrally, 5 prime genes later and more caudally. Each arch therefore ends up with a unique combination of Hox proteins, the pharyngeal Hox code, which is an address: it tells arch 1 to build maxilla, mandible, malleus and incus with trigeminal innervation, and arch 2 to build Reichert's cartilage, stapes and styloid process with facial innervation. The code is combinatorial and positional, not dorsoventral, not random and not left-right asymmetric.

Rule Because identity is set before migration, losing a coding factor does not merely shrink an arch, it can respecify it, and failure of a stream removes the entire skeletal set of that arch. Hit the crest and the arch is misbuilt even though its pouch and cleft are present.

Knockouts: the level of the gene predicts the level of the defect

Gene	Position in the hierarchy	Knockout phenotype
SHH	Upstream of the Hox code. Secreted by notochord, prechordal plate and later pharyngeal endoderm; required to establish Hox expression and to keep arch crest alive and proliferating.	Headless embryos; the rostral programme is never built. The milder human end of the same spectrum is holoprosencephaly: hypotelorism, a single central incisor, at the extreme cyclopia with a proboscis. This is also why cholesterol synthesis defects cause midline craniofacial disease.
Hoxa-1 / Hoxb-1	Most rostral Hox domain of the hindbrain, covering the rhombomeres whose crest and mesoderm build the ear region	No ears. The ear is a sensitive readout because it needs cleft 1, pouch 1, and the cartilages of arch 1 (malleus, incus) and arch 2 (stapes). In humans, microtia is likewise a common sign of first and second arch disruption.
Hoxa-3	More caudal Hox domain, carried by crest into arches 3 and 4	No thymus and no parathyroids, the mouse counterpart of DiGeorge syndrome.

Memory hook Hierarchy: SHH turns on the Hox code -> crest carries the code into the arch -> crest builds the arch. Read the number: rostral Hox defects give ear and jaw anomalies, caudal ones give pouch 3 and 4 failure plus great vessel defects.

Rule Hox expression is dose dependent, a Goldilocks window. Too little leaves an arch with no address and it fails to build its derivatives (Hoxa-1 ear, Hoxa-3 pouch 3 and 4). Too much, as high-dose retinoic acid produces, switches genes on in arches where they do not belong and shifts boundaries caudally, so an arch builds its neighbour's structures. Both ends harm, which is why vitamin A deficiency and vitamin A excess are both teratogenic. Hox proteins are not chemically toxic; the damage is mispatterning, not poisoning.

Trap Keep the pathways separate. Pouch endoderm does provide important arch signals such as FGF and SHH, but the Hox positional code itself is imported by migrating crest. SHH signals through Patched and Smoothened at the membrane; retinoids act at nuclear receptors. SHH lies upstream of Hox, so excess Hox cannot switch SHH off. A dorsoventral gradient (SHH from notochord and floor plate versus BMPs from the roof plate) patterns neural tube height, not arch identity; left-right asymmetry is nodal cilia with Nodal, Lefty and Pitx2, a separate axis system.

8. Teratogens: why the neural crest is the vulnerable cell

High yield Neural crest cells are migratory, highly proliferative and metabolically active, and they do all of that while lacking superoxide dismutase and catalase. They cannot convert superoxide to hydrogen peroxide and then to water, so any insult that raises reactive oxygen species kills them preferentially. Because crest supplies the skeleton, connective tissue and patterning of every arch and also septates the cardiac outflow tract, crest death translates directly into craniofacial and conotruncal malformation. Treat any craniofacial teratogen question as a question about crest survival.

Trap The gap is specifically in free-radical scavenging. Crest cells do have DNA repair enzymes, they migrate through vascularized mesenchyme so they are not avascular, and they divide rapidly rather than slowly, which is part of what makes oxidative injury so costly.

Teratogen	Mechanism	Timing and exposure	Phenotype
Alcohol	Reactive oxygen species ONLY. Ethanol metabolism generates ROS; crest cells cannot detoxify them and die. Alcohol binds no nuclear receptor and never touches Hox promoters, so there is no transcriptional over-activation. The arches are depleted of crest rather than mispatterned.	No safe threshold and no safe trimester. Crest loss in the frontonasal prominence and arch 1 flattens the philtrum and midface; apoptosis in the developing brain causes the microcephaly.	Fetal alcohol syndrome: smooth philtrum, thin vermilion upper lip, short palpebral fissures (the three diagnostic facial features) plus microcephaly and intellectual disability. The leading preventable cause of intellectual disability.
Retinoic acid / isotretinoin (13-cis-retinoic acid, a vitamin A analog)	Two hits. (1) Its metabolism generates ROS in antioxidant-poor crest, killing the cells. (2) It is small and lipophilic, crosses the membrane, binds nuclear RAR and RXR; the dimer binds RAREs (retinoic acid response elements) in the promoters and enhancers of Hox genes and switches them on directly, hyperactivating the Hox code so surviving crest builds the wrong structures in the wrong arch. Control is transcriptional and direct, so the retinoid dose is a dial on the Hox code.	Exposure during the arch window. The ear is hit because it needs arch 1 and 2 crest plus cleft 1 and pouch 1; the palatal shelves fail to fuse; the crest that septates the outflow tract fails. Prescribing requires two forms of contraception and monthly pregnancy testing.	Isotretinoin embryopathy: microtia or anotia, cleft palate, conotruncal heart defects, central nervous system anomalies. At very high dose in mouse embryos the phenotype phenocopies Treacher Collins: arch 1 and 2 crest loss with malar and mandibular hypoplasia, malformed ears and downslanting eyes.

Memory hook Alcohol = ROS only. Retinoic acid = ROS plus Hox hyperactivation. That contrast is the whole comparison. Clinically: alcohol reads philtrum, lip, palpebral fissures, microcephaly; retinoid reads ear plus palate plus outflow tract, classically after an acne drug in pregnancy.

Rule A teratogen and a gene converge when they hit the same cell population. High-dose retinoic acid and TCOF1 mutation both deplete arch 1 and 2 crest, so they look alike. Classify craniofacial syndromes by which arch's crest failed, and read the face: midface, jaw and ears equals arches 1 and 2; calcium and T cells equals pouches 3 and 4.

Trap Other teratogens have different signatures and different mechanisms. Phenytoin: fetal hydantoin syndrome, cleft lip and palate, digital and nail hypoplasia, growth restriction. Lithium in the first trimester: Ebstein anomaly, cardiac not craniofacial. Tetracycline: stains developing teeth and deposits in bone, no arch or outflow tract malformation. Valproate and antifolates such as methotrexate: folate interference, neural tube defects such as spina bifida, not pure crest loss. Folate antagonism is never the mechanism of alcohol or retinoids, and a teratogen cannot excise a chromosomal segment such as 22q11.

9. The syndrome table

Condition	Defect	Gene or cause	Key findings
Treacher Collins (mandibulofacial dysostosis)	Arch 1 and 2 neural crest apoptosis. Treacle is needed for ribosome biogenesis in rapidly proliferating crest; without it the crest destined for arches 1 and 2 dies, so every arch 1 skeletal derivative is underbuilt. Symmetric. Spares the larynx, because arch 4 and 6 cartilage is lateral plate mesoderm.	TCOF1 (Tcof-1), encoding treacle	Malformed or clefted zygomatic bones with flat cheeks; mandibular hypoplasia / micrognathia from poor membrane ossification around Meckel's cartilage; downslanting palpebral fissures with lower-eyelid colobomas following the deficient malar bone; malformed external ears with conductive hearing loss from ossicle defects. No hypocalcemia, no thymic aplasia, no pigmentary sign.
DiGeorge (CATCH-22)	Failure of pouches 3 and 4 plus the crest of arches 3, 4 and 6. The same crest septates the cardiac outflow tract. Locked in by about week 10, the window in which gland migration completes.	22q11.2 microdeletion with loss of TBX1. Mouse counterpart: Hoxa-3 knockout.	Cardiac defects (conotruncal: truncus arteriosus, tetralogy of Fallot) and aortic arch anomalies from arch 4; Abnormal facies from arch crest deficiency; Thymic hypoplasia with T cell deficiency and recurrent candidiasis (pouch 3 ventral); Cleft palate with velopharyngeal insufficiency; Hypocalcemia from parathyroid hypoplasia (pouch 3 and 4 dorsal wings) presenting as neonatal tetany and hypocalcemic seizures. Every feature is a pouch 3-4 or arch crest derivative, so the list can be rebuilt rather than memorized. Conductive hearing loss occurs in some patients from palatal dysfunction but is not one of the five CATCH features; cataracts and hyperthyroidism are not features at all.
Branchial cleft cyst / sinus / fistula	Persistent cervical sinus: the ectoderm-lined space left when arch 2 overgrows clefts 2 to 5 and fuses with the epicardial ridge fails to obliterate. Trapped squamous epithelium keeps secreting.	Developmental, not genetic; a persistent ectodermal remnant along the path of cleft 2	Painless smooth lateral neck mass at the anterior border of the sternocleidomastoid near the mandibular angle; does not move on swallowing or tongue protrusion; typically presents in late childhood and enlarges after an upper respiratory infection. Cyst = no opening; sinus = one; fistula = two, running tonsillar fossa to skin between the carotids, with intermittent saliva-like drainage. Excise completely.
Waardenburg (type 1)	Neural crest migration and differentiation disorder, hitting melanocyte precursors and the facial prominences. A crest disorder that is pigmentary rather than skeletal.	PAX3	White forelock, patchy hypopigmentation, heterochromia iridis; congenital sensorineural deafness from loss of melanocytes in the cochlear stria vascularis; dystopia canthorum with apparent hypertelorism; sometimes cleft lip or palate. Any disorder combining pigment, hearing and facial patterning is a crest disorder, because those tissues share one origin.
Neurofibromatosis	Neoplasia of neural crest derivatives. Schwann cells, perineurial and fibroblastic cells of the nerve sheath, and melanocytes are all crest.	NF1 inactivates neurofibromin, a RAS GTPase-activating protein, so loss of function leaves RAS signalling on and crest-derived cells proliferate.	Neurofibromas and plexiform neurofibromas (nerve sheath); cafe-au-lait macules and axillary freckling (melanocytes); Lisch nodules as pigmented iris hamartomas; in NF2, Schwann cell vestibular schwannomas. Crest neoplasia family: pheochromocytoma, neuroblastoma, medullary thyroid carcinoma.
Fetal alcohol syndrome	Crest death from reactive oxygen species; crest depleted rather than mispatterned	Prenatal alcohol exposure (no safe dose, no safe trimester)	Smooth philtrum, thin upper lip, short palpebral fissures, microcephaly, intellectual disability.
Isotretinoin embryopathy	Crest death from ROS plus Hox hyperactivation via RAREs	Isotretinoin, a vitamin A analog used for severe acne; one of the most potent known human teratogens	Microtia or anotia, cleft palate, conotruncal heart defects, CNS anomalies. At very high dose, a Treacher Collins-like malar and mandibular hypoplasia.

Trap Other named decoys worth separating from the arch syndromes: holoprosencephaly and cyclopia come from deficient SHH signalling and midline forebrain induction, a different pathway and a different axis of failure; PAX6 mutation causes aniridia and other anterior eye segment defects, acting on the optic and lens field rather than on migrating crest; tooth agenesis reflects reciprocal epithelial-mesenchymal signalling in arch 1 involving PAX9 and MSX1, not rostral Hox loss.

Rule Localize every arch syndrome by asking which population failed. Crest of arches 1 and 2 gives midface, jaw, eyelid and ear anomalies. Endoderm of pouches 3 and 4 gives calcium and T cell failure. Crest of arches 3, 4 and 6 gives conotruncal and great vessel defects. Ectoderm of cleft 2 gives a lateral neck cyst or fistula.

10. If you remember nothing else

High yield Arches 1-2 by weeks 3-3.5; crest migrates in at weeks 4.5-5; arches 3, 4 and 6 at week 5; each arch gets its own nerve and artery at weeks 5.5-6; Meckel's cartilage gone by week 8; thymus and parathyroids landed by week 10. The series is 1, 2, 3, 4, 6 and there is never an arch 5.

High yield One arch, one nerve: 1 = V3, 2 = VII, 3 = IX, 4 = X via superior laryngeal, 6 = X via recurrent laryngeal. The nerve follows the muscle wherever it migrates, so a scattered cluster of deficits localizes to a single arch. CN XII and CN V2 are not arch motor nerves.

High yield CAP: Clefts are ectoderm and outside, Arches are mesoderm, Pouches are endoderm and inside. Crest builds skeleton, mesoderm builds muscle, endoderm builds glands.

High yield Cartilage source splits at arch 4: arches 1-3 are neural crest, arches 4 and 6 are lateral plate mesoderm. Meckel's is a template only, so the mandible and midface are membrane bone from the surrounding crest while only malleus and incus ossify endochondrally. Ossicles run 1-1-2.

High yield Pouches: Ear, Tonsils, Bottom, Top. Pouch 3 dorsal is the inferior parathyroid and pouch 3 ventral is the thymus; pouch 4 dorsal is the superior parathyroid; pouch 4/5 ventral (ultimobranchial body) gives the calcitonin-secreting C cells. The crossover happens because the descending thymus tows the pouch 3 gland below the pouch 4 gland.

High yield Only cleft 1 survives, as the external auditory meatus, and only pouch 1 touches a cleft, making the three-layered tympanic membrane. Clefts 2-4 are sealed into the cervical sinus; a persistent sinus is a lateral branchial cleft cyst, lateral and anterior to the sternocleidomastoid, and it does not move on swallowing.

High yield Crest carries the Hox code in from its rhombomere: R1-R2 plus midbrain to arch 1 (OTX2, Hox-free), R4 to arch 2, R6-R7 to arch 3, R8 to arches 4 and 6. SHH is upstream of Hox. Hox dose is a Goldilocks window: too little leaves an arch unlabelled, too much scrambles the code.

High yield Neural crest lacks superoxide dismutase and catalase, so it dies first to any oxidative insult. Alcohol = ROS only (smooth philtrum, thin upper lip, short palpebral fissures, microcephaly). Retinoic acid = ROS plus Hox hyperactivation via RAREs (microtia or anotia, cleft palate, conotruncal defects).

High yield Two syndromes, two territories. TCOF1 -> arch 1 and 2 crest death -> Treacher Collins: small jaw, flat zygoma, lower-eyelid coloboma, bad ears. 22q11.2 / TBX1 -> pouches 3 and 4 -> DiGeorge / CATCH-22: hypocalcemic seizures, absent thymus with candidiasis, conotruncal heart defect, cleft palate, abnormal facies.
